研究目的：

1.通过提高慢病用药报销政策知晓度与参与率，降低在“基本公卫规范服务管理”系统内的高血压和糖尿病患者慢病用药负担。 2.通过个性化针对性的教育和随访等手段增加患者就诊次数和服药依从性，进而更好地控制患者血压血糖。  

Objectives of Study：

1. By increasing the awareness and participation rate of chronic disease medication reimbursement policies, reduce the burden of chronic disease medications for hypertension and diabetes patients in the "Basic Public Health Standardized Service Management" system. 2. Increase the number of patient visits and medication compliance by means of personalized targeted education and follow-up, so as to better control the patient's blood pressure and blood sugar.

纳入标准：

所有在村诊室工作的医生均符合纳入标准。在村诊室负责慢性病管理的村医将承担主要职责。

Inclusion criteria

All village doctors under the village clinics. The village doctor who is responsible for non-communicable diseases (NCDs) management takes on main responsibility.

排除标准：

年龄过大的乡村医生，没有计算机使用基础。

Exclusion criteria：

Over-aged village doctors, no basis for computer use.

31个村庄将被纳入研究。一个村庄将被随机去除，剩余30个村庄将根据计算机程序产生的随机数字表、根据所在镇分层、随机被分配到干预组和对照组，分配比例是1:1。

31 villages are included in the study. 1 village is randomly dropped, and the rest of the villages will be randomly assigned to the intervention and control group stratified by township with an allocation ratio of 1:1 based on random numbers generated by the computer.

According to the progress of the research, raw research data should be made freely available to all researchers in specific ways.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

研究者根据受试者的原始观察记录，将数据及时、完整、正确、清晰地载入病例报告表，录入采用相应的数据库系统双人双机录入，之后对数据库进行两遍对比。电子数据文件分类保存，并有多个备份保存于不同磁盘或记录介质上，妥善保存，防止损坏。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Based on the oringinal observation records, researchers will completely write accurate data into case report forms in time. Input the data into corresponding database system by two special researchers with two computers respectively. After that, researchers compare two database twice and electronic data will be conserved and backup.
