探究胃食管结合部癌中保留胃窦的空肠间置双通道术式与传统消化道重建术式（全胃Roux-en-Y术式重建，残胃食管吻合式重建）对患者术后生存质量及预后的影响  

Objectives of Study：

To ivestgate the long-term quality of life and prognosis comparison between antrum-preserving double tract reconstruction of jejunal interposition procedure and traditional digestive reconstruction in proximal gastrectomy for adenocarcinoma of the esophagogastric junction.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄18-70岁；2.胃镜证实肿瘤位于胃食管结合部（Seiwert Ⅰ,Ⅱ,Ⅲ）；3.病理检查证实为腺癌4.胸片/胸部CT、腹部B超/腹部CT除外有远处转移；5.肿瘤可通过手术进行切除；6.无胃肠道手术病史；7.无严重心肺功能障碍；8.预计生存时间超过六个月；9.患者Karnofsky评分大于等于70，Zubrod-ECOG-WHO评分大于等于2；10.WBC > 3.5×109/ml, PLT> 10×109/ml, 总血清胆红素< 22.2 umol/l,BUN <7.14mmol/l, 肌酐< 132umol/l或肌酐清除率> 50ml/min； 11.患者必须签署知情同意书；12.无其他重要相关疾病（例如其他肿瘤，严重心脏和中枢神经系统疾病等)；

Inclusion criteria

1. Age between 18 and 70; Gastroscopy:
2. tumor locate at the gastroesophageal junction (Seiwert I,II, III);
3. Pathological diagnosis of adenocarcinoma;
4. CXR/CT, abdominal ultrasound/CT exclude metastasis;
5. Resectable tumor;
6. No history of previous gastrointestinal surgery;
7. Good cardiopulmnoary functions;
8. Expectation survival period more than 6 months;
9. Karnofsky score more than 70 while Zubrod-ECOG-WHO score beyond 2;
10. WBC > 3.5*10^9/ml, PLT> 10*10^9/ml, T-Bil< 22.2 umol/l, BUN <7.14mmol/l, Cr<132umol/l or creatinine clearance rate > 50ml/min;
11. Patients included in this trail must be informed;
12. No other relevant diseases (ex: tumors in other sites or serious disease heart or central nervous system, CNS).

排除标准：

1.胃良性肿瘤；既往或同时存在其它恶性肿瘤病史；2.MRI／EBUS／CT提示局部不可切除病变；3.不能耐受全麻或手术治疗；4.不能控制的内科疾病（如不稳定心绞痛）；5.曾经接受过免疫抑制（包括皮质类固醇）、细胞毒性药物、放射治疗；6.严重的未控制的感染,以及严重的过敏史或过敏体质者；7.影响认知能力的神经或精神异常，包括中枢神经系统转移；8.严重肺部或心脏疾病病史者，如急性心梗发作小于6个月、慢性心衰、有症状的心绞痛，凝血功能显著异常等。

Exclusion criteria：

1. Benign neoplasm of stomach;
2. Past history or excisting other malignancy;
3. MRI/EBUS/ CT suggest unresectable tumors;
4. Difficulty in endure anesthesia and surgery;
5. Uncontroled underlying diseases (unstable angina etc.);
6. Previous history of immunosuppresive therapy (including corticosteroids therapy), cytotoxic chemotherapy or radiotherapy;
7. Serious uncontroled infections or serious allergy history;
8. Diseases of CNS or psyche which affect congnition including CNS metastasis;
9. Serious pulmonary heart disease such as: acute myocardial infarction shorter than 6 months or congestive heart failure or symptomatic angina, coagulative function abnormal etc.
