研究目的：

将优化后的智能药物管理系统应用于老年慢性病患者住院、居家期间，并评价其应用效果，探讨该系统对老年慢性病患者的用药依从性、用药知识及合理用药自我效能的影响。  

Objectives of Study：

The purpose of this study is to apply the optimized intelligent drug management system to older adults with chronic diseases during hospitalization and home stay, and to evaluate its application effect, and to explore the influence of the system on medication compliance, medication knowledge and rational drug use self-efficacy of older adults with chronic diseases

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄≥60岁；
②至少患有一种慢性病；
③服用两种及以上药物，服药时间至少半年以上（包括处方药和非处方药）；
④本人及家属知情同意

Inclusion criteria

1. Aged >=60 years old;
2. suffering from at least one chronic disease;
3. taking two or more drugs for at least half a year (including prescription drugs and non prescription drugs);
4. informed consent of myself and his family members.

排除标准：

①有精神障碍或认知障碍；②严重听力障碍者；③患有重症或终末疾病

Exclusion criteria：

① Mental disorders or cognitive impairment; ② severe hearing impairment; ③ severe or terminal disease
