研究目的：

对传统肱骨小夹板设计中的力学缺陷进行改良，对比论证改良型肱骨小夹板在治疗肱骨干骨折时的临床疗效并分析其力学效应。  

Objectives of Study：

Objective to improve the mechanical defects in the design of traditional humeral splint, to compare and demonstrate the clinical efficacy of modified humeral splint in the treatment of humeral shaft fracture, and to analyze its mechanical effect.

药物成份或治疗方案详述：

两组均采用相同手法进行整复，纠正重叠 、旋转、侧方、成角移位后，先将上臂用绷带轻松缠绕数层，再根据骨折的移位情况放置相应的平垫。对照组：采用常规小夹板固定，三角巾中立位悬吊患肢于胸前。实验组：采用改良夹板，先根据患肢长度对常规小夹板长度进行适度裁剪，然后在常规4块小夹板中的前、后、外侧夹板上端两侧上端各缝直径约0.8cm绳圈。前、后、外侧夹板上端超肩关节，利用6个绳圈穿绳分别打结均衡固定3块夹板，并将绳绕对侧腋窝（腋窝下垫厚棉垫）以固定肩关节，内侧夹板上端至腋窝下平面，4块夹板下端平尺骨鹰嘴，上臂部夹板以3 条扎带捆扎，松紧以上下活动1cm为宜。再加用L型直角板，上达肩关节，下至掌指关节，确定肘关节屈曲90度，再用“8”字绷带固定法维持其位置，然后用三角巾悬吊患肢于胸前。横断或断端有分离骨折的，为了防止分离移位，采用弹力绷带缠绕肩、肘部以对抗远折端肢体重心的牵引力，一般3 周后拆除L型夹板及弹力绷带，改用常规上臂小夹板外固定，以利肘关节和肩关节的功能活动锻炼。两组固定后注意检查患肢远端血运、感觉与活动情况，在夹板固定后就开始握拳、等张肌力训练等渐进式功能锻炼，后期逐步加强肩、肘功能锻炼。两组均定期拍 x光片复查，3周后进行患侧及健侧的肱骨干CT检查。两组均在夹板固定完成后在肩关节处安放位置传感器，监测肩关节活动度。 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄 18-75 岁，性别不限；
②确诊为肱骨干骨折患者，符合经典OTA/AO分型，A1-C1型，闭合性、新鲜性骨折；
③自愿接受中医内外治疗方法；
④全程参与本次试验，并签署知情同意书。

Inclusion criteria

① The age is 18-75 years old, and the gender is not limited;
② The diagnosis of humeral shaft fracture was in accordance with the classic OTA / AO classification, a1-c1, closed and fresh fracture;
③ Willing to accept internal and external treatment of traditional Chinese medicine;
④ Participate in the whole experiment and sign the informed consent.

排除标准：

①开放性骨折，合并神经、血管和肌腱损伤；
②病理性或者陈旧性骨折；
③要求达到解剖复位者；
④合并严重心脑肾及内分泌系统相关疾病，不能耐受手法复位和夹板固定。

Exclusion criteria：

① Open fracture with nerve, blood vessel and tendon injury;
② Pathological or old fracture;
③ Anatomical reduction is required;
④ Combined with severe heart, brain, kidney and endocrine system related diseases, can not tolerate manual reduction and splint fixation.

Blinding：

In this trial, both the outcome assessors and the statistical analyst will be blinded to the randomization allocation and will not be involved in performance of the interventions. It will be impossible to blind the participants and practitioners since they will perform the interventional treatment. Unblinding will only occur once all the outcome data collection is completed.
