研究目的：

1、主要目的： 在社区高危人群（高血压、糖尿病、冠心病患者）中，探究HHE联合BNP主动快速筛查策略能否提高心衰分期的检出率 探究心衰主动筛查项目的开展，能否提高心衰药物规范化使用率 2、次要目的： 调查社区医生识别心衰的能力;  

Objectives of Study：

1. Major objective To assess the ability and feasibility of the combination of hand-held echocardiography and natriuretic peptide to screen heart failure in patients with high risk factors in Chinese communities. To explore whether the positive HF screening program could improve the use of the guideline directed medical therapy 2. Minor objective To assess the general physicians (GPs) abilities to recognize heart failure

纳入标准：

1. 35岁以上
2. 伴有心衰危险因素，如高血压、糖尿病、肥胖
3. 同意参加研究

Inclusion criteria

1. Patients over 35 years old;
2. Patients with heart failure risk factors, such as hypertension, diabetes, obesity.
3. Patients who agreed to participate in the study.

排除标准：

1. 已知患有心衰
2. 不愿意参加本研究

Exclusion criteria：

1. Those patients with known heart failure;
2. Patients who are not willing to participate in this study.
