研究目的:

β-羟基-β-甲基丁酸钙(HMB)可有效增加疗养院非卧床老年人的体重、去脂体重、肌肉质量及握力,且可增加肿瘤终末期及艾滋病患者的体重及去脂体重。但未见HMB用于HCVD患者的报道,用于ICU患者的仅有两项报道。HMB可改善ICU严重创伤患者的全身炎症反应,减少氮流失;可减少老年卧床COPD患者肌肉分解。神经病变可造成或加速肌肉减少症的发生,肌少症则与老年人骨折风险的增加有关,将增加死亡风险及疾病负担。因此预防出血性脑血管病患者的肌肉组织丢失可能减少卒中后髋关节骨折的发生。为此本项目拟观察HMB降低HCVD患者炎症反应的发生,减少患者氮流失改善氮平衡,从而预防肌肉组织丢失的效果,优化出血性脑血管病患者营养治疗方案,从而改善临床结局,降低患者与家属及社会负担。  

Objectives of Study:

Calcium β-hydroxy-β-methylbutyrate (HMB) can effectively increase the weight, fat-free body weight, muscle mass and grip strength of ambulatory elderly in nursing homes, and can increase the weight and fat-free body weight of patients with end-stage tumors and AIDS. However, there were no reports of HMB used in HCVD patients, and there were only two reports used in ICU patients. HMB can improve the systemic inflammatory response in severely traumatized ICU patients and reduce nitrogen loss; it can reduce muscle decomposition in elderly patients with COPD in bed. Neuropathy can cause or accelerate the occurrence of sarcopenia, and sarcopenia was related to the increased risk of fracture in the elderly, which will increase the risk of death and the burden of disease. Therefore, preventing the loss of muscle tissue in patients with hemorrhagic cerebrovascular disease may reduce the occurrence of hip fractures after stroke. This project intends to observe the effect of HMB to reduce the occurrence of inflammatory reactions in HCVD patients, reduce nitrogen loss in patients and improve nitrogen balance, thereby preventing muscle tissue loss, and optimizing the nutritional treatment plan for patients with hemorrhagic cerebrovascular disease, thereby improving clinical outcomes and reducing patient Family and social burdens.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

1)受试者或受试者的法律代表自愿签署知情同意书;
2)年龄20-70周岁;
3)血流动力学稳定,且有营养不良风险;
4)肠内营养支持或能经口进食者。

Inclusion criteria

1. The informed consent is signed voluntarily by the subject or the legal representative of the subject;
2. Subjects aged 20-70 years old;
3. Subjects with stable hemodynamics and risk of malnutrition;
4. Subjects with enteral nutrition support or oral feeding.

排除标准:

1)Hunt-Hess分级V级;
2)格拉斯哥(GCS)评分≤5分;
3)肾功能严重受损者(e-GFR<30ml/min);
4)肝衰竭、失代偿性慢性肝病、活动性乙型肝炎、接受治疗的丙型肝炎或肝性脑病患者;
5)慢性心肺功能不全(NY心力衰竭分级在III、IV级);
6)患有活动性结核病、HIV且CD4细胞计数<350/mm3并且使用稳定抗逆转录病毒治疗方案(根据病历记录)小于3个月者;
7)患有无法摄取或吸收试验产品的胃肠道疾病(如:胃旁路、炎性肠病、乳糜泻、短肠综合征、急性/慢性胰腺炎、胃肠道出血等);
8)血糖控制不佳的糖尿病患者(糖化血红蛋白> 8%);
9)患有需要免疫抑制剂治疗的自身免疫性病;
10)无法接受管饲喂养者;
11)原因未明脑出血者;
12)存在主要研究者或研究医生认为不适宜纳入的任何其他状况或事件者;
13)对研究产品中的任何成分表示过敏或不耐受者;
14)正在进行另一项临床研究者;
不愿意签订知情同意书者。

Exclusion criteria:

1. Hunt Hess grade V subjects;
2. Subjects with GCS score ≤ 5;
3. Subjects with severe impairment of renal function (e-gfr < 30ml / min);
4. Patients with liver failure, decompensated chronic liver disease, active hepatitis B, treated hepatitis C or hepatic encephalopathy;
5. Subjects with chronic cardiopulmonary insufficiency (NY heart failure grade III and IV);
6. Patients with active tuberculosis, HIV, CD4 cell count < 350 / mm3 and using stable antiretroviral therapy (according to medical records) for less than 3 months;
7. Patients with gastrointestinal diseases (e.g. gastric bypass, inflammatory bowel disease, celiac disease, short bowel syndrome, acute / chronic pancreatitis, gastrointestinal bleeding, etc.) who cannot ingest or absorb the test products;
8. Diabetic patients with poor glycemic control (HbA1c > 8%);
9. Patients with autoimmune diseases requiring immunosuppressive therapy;
10. Patients who could not accept tube feeding;
11. Patients with cerebral hemorrhage of unknown cause;
12. Subjects with any other conditions or events that the principal investigator or study physician considers inappropriate for inclusion;
13. Subjects who are allergic or intolerant to any component in the study product;
14. Subjects in another clinical study;
15. Subjects who are not willing to sign informed consent.
