研究目的：

评价复方曲肽注射液用于急性缺血性卒中患者的有效性和安全性。  

Objectives of Study：

To evaluate the efficacy and safety of Compound Troxerutin and Poreine Cerebroside Injection in patients with acute ischemic stroke.

药物成份或治疗方案详述：

药物成份：复方曲肽注射液含有曲克芦丁、活性多肽、多种氨基酸、多种神经节苷脂等活性物质。 治疗方案：试验组：基于指南的规范化治疗+复方曲肽注射液；对照组：基于指南的规范化治疗+安慰剂（0.9%氯化钠注射液）。复方曲肽注射液/安慰剂，一次10mL加入0.9%氯化钠注射液250mL中，缓慢滴注，一日一次，连续用药10日。 

Description for medicine or protocol of treatment in detail：

Ingredients: Compound Troxerutin and Poreine Cerebroside Injection contains troxerutin, active peptides, multiple amino acids, multiple gangliosides and other active substances. Treatment plan: experimental group: standardized treatment based on guidelines + Compound Troxerutin and Poreine Cerebroside Injection; control group: standardized treatment based on guidelines + placebo (0.9% sodium chloride injection). Compound Troxerutin and Poreine Cerebroside Injection/placebo, 10 mL each time, add to 250 mL of 0.9% sodium chloride injection, instill slowly, once a day for 10 consecutive days. 

纳入标准：

1）符合急性缺血性卒中的诊断；
2）发病72h以内；
3）4分≤NIHSS评分≤16分，且NIHSS第五项上肢和第六项下肢评分之和≥2分；
4）18≤年龄≤80；
5）受试者或其监护人知情同意，并签署知情同意书。

Inclusion criteria

1) Meet the diagnosis of acute ischemic stroke;
2) Within 72h of onset;
3) 4 points <= NIHSS score <= 16 points, and the sum of the fifth upper limb and sixth lower limb scores of NIHSS >= 2 points;
4) 18 <= age <= 80 years;
5) Subjects or their guardians informed consent and signed an informed consent form.

排除标准：

1）已接受早期再通（溶栓或血管内治疗）的患者；
2）由脑肿瘤、脑外伤、血液病等引起的卒中患者；
3）既往有卒中病史且遗留后遗症严重影响结局测评者，即本次卒中发病前改良Rankin量表得分≥2；
4）合并有其他影响肢体活动功能的疾病，治疗前合并有跛行、骨关节炎、类风湿性关节炎、痛风性关节炎等引起的肢体活动功能障碍影响神经功能检查者；
5）合并严重肝、肾、心功能不全者（备注：肝功能不全是指ALT 或AST 值大于2.5倍正常上限；肾功能不全是指血肌酐值大于2.5倍正常上限；心功能不全是指左心室射血分数低于50%。）；
6）明确合并房颤或发病机制考虑为心源性栓塞的缺血性卒中患者；
7）过敏体质或对研究药物过敏者；
8）患有其他严重疾病，预期生存时间小于3 个月者；
9）大面积梗塞伴严重意识障碍（NIHSS 量表1a 项得分≥2 分）或痴呆，或具有其它精神损害，研究者认为不适于参加研究；
10）遗传性糖脂代谢异常（神经节苷脂累积病，如：家族性黑蒙性痴呆、视网膜变性病）患者；
11）妊娠或哺乳期妇女、精神病患者；
12）近1个月参加其他临床试验者。

Exclusion criteria：

1) Patients who have received early recanalization (thrombolysis or endovascular treatment);
2) Patients with stroke caused by brain tumors, brain trauma, blood diseases, etc.;
3) Those who have a history of stroke and whose sequelae seriously affect the outcome evaluation, that is, the modified Rankin scale score >= 2 before the onset of the stroke;
4) Complicated with other diseases that affect the function of the limbs, combined with limb movement dysfunction caused by claudication, osteoarthritis, rheumatoid arthritis, gouty arthritis, etc. before treatment, affecting the neurological function examination;
5) Patients with severe liver, kidney, and cardiac insufficiency (Note: Liver insufficiency means that the ALT or AST value is greater than 2.5 times the upper limit of normal; renal insufficiency means that the blood creatinine value is greater than 2.5 times the upper limit of normal; cardiac insufficiency means left The ventricular ejection fraction is less than 50%.);
6) It is clear that patients with ischemic stroke combined with atrial fibrillation or the pathogenesis considered as cardiogenic embolism;
7) Patients with allergies or allergic to research drugs;
8) Suffer from other serious diseases and the expected survival time is less than 3 months;
9) Large-area infarction with severe disturbance of consciousness (NIHSS scale 1a item score >= 2 points) or dementia, or other mental impairment, the researcher considers not suitable for participating in the study;
10) Patients with inherited abnormal glucose and lipid metabolism (ganglioside accumulation diseases, such as familial amaurian dementia, retinal degeneration);
11) Pregnant or breastfeeding women, mental patients;
12) Participants in other clinical trials in the past month.
