研究目的：

1.评估氧化苦参碱（OMT）治疗脓疱性银屑病或红皮病型银屑病的疗效。 2.观察OMT对脓疱型银屑病和红皮病型银屑病患者血清抗氧化及血液流变学指标的影响。 3.观察OMT对脓疱型银屑病和红皮型银屑病患者血清免疫和炎症细胞因子水平的影响。 4.观察脓疱性银屑病或红皮病型银屑病皮损OMT治疗前后的组织病理学变化。应用免疫组织化学方法观察对皮损表皮细胞增殖和凋亡的影响。  

Objectives of Study：

1．To assess the efficacy of oxymatrine (OMT) for the treatment of pustular psoriasis or erythrodermic psoriasis. 2．To measure the effect of OMT on serum antioxidant and blood rheology indices in patients with pustular psoriasis or erythrodermic psoriasis; 3．To measure the effect of OMT on levels of serum immune and inflammatory cytokines in patients with pustular psoriasis or erythrodermic psoriasis; 4．To examine histopathological changes before and after treatment with OMT in patient skin lesions with pustular psoriasis or erythrodermic psoriasis. To observe the effects on epidermal cell proliferation and apoptosis by immunohistochemistry in skin lesions.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.红皮病型银屑病，体表面积（BSA）评分≥75%，病程≥6个月，且至少接受过一次全身治疗。
2.脓疱性银屑病，银屑病病程≥6个月，且至少有1个系统治疗疗程。

Inclusion criteria

1. Erythrodermic psoriasis, Body Surface Area (BSA) score >=75%, course of psoriasis >=6 months, and at least one previous course of systemic treatment;
2. Pustular psoriasis, course of psoriasis >=6 months, and at least one previous course of systemic treatment.

排除标准：

1.对氧化苦参碱过敏。
2.有严重肝肾损害、精神疾病、造血功能障碍或其它严重器质性疾病者。
3.对于脓疱型银屑病或红皮病型银屑病患者，在过去8周内给予免疫抑制、大剂量糖皮质激素或维甲酸治疗，并在4周内使用外用药物。
4.怀孕和哺乳。

Exclusion criteria：

1. Allergy to oxymatrine;
2. Presence of severe liver and kidney damage, mental illness, hematopoietic dysfunction, or other serious organic disease;
3. For patients with pustular psoriasis or erythrodermic psoriasis, treatment with immunosuppression, high doses of glucocorticoids or retinoid in the past 8 weeks and used topical drugs in the 4 weeks.
4. Pregnancy and lactation.
