研究目的：

明确上海社区老年人群生活方式、慢性基础疾病、步态、语音等的人群分布特征及3年内的疾病转化率，探寻老年期脑高发病的影响因素，寻找疾病转化的早期预警信号，建立疾病的风险预测模型，为后续的脑血管病、AD和PD专病队列研究奠定基础。  

Objectives of Study：

To clarify the population distribution characteristics of lifestyle, chronic basic diseases, gait, voice, etc. of the elderly population in Shanghai community and to explore their influence on the high incidence of brain diseases(Cerebrovascular disease, Alzheimer's disease and Parkinson's disease) in the elderly, and to look for early warning signals of disease transformation, and to establish the disease risk prediction model accordingly which will lay the foundation for cerebrovascular disease, AD and PD special disease cohort research in the future.

纳入标准：

(1) 年龄在55岁以上（含55岁）；
(2) 长期居住上海；
(3) 能够理解知情同意书，自愿参与并签署知情同意书

Inclusion criteria

(1) Aged 55 and above;
(2）Long-term residence in Shanghai;
(3) Able to understand the informed consent, participate voluntarily and sign the informed consent

排除标准：

（1）盲聋哑人、植物人；
（2）有严重或不稳定的其他内科疾病可能影响脑功能或影响对认知功能的评价；
（3）拒绝参与本项研究。

Exclusion criteria：

(1) The blind, deaf-mute and vegetative patients;
(2) Other serious or unstable medical conditions that may affect brain function or affect the evaluation of cognitive function;
(3) Refuse to participate in this study.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

根据研究目的及方案要求，设计必要的问卷调查表以收集受试者的数据，并定义研究流程、数据变量说明及其收集的数据项，同时形成相应的问卷调查员填写指南。经审查和批准后定稿，其过程要有版本管理。 本项目的数据管理将在讯飞开发的同济脑健康的一体化服务平台上运行。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

According to the research purpose and program requirements, necessary questionnaires were designed to collect the data of the subjects, and the research process, data variable description and data items collected were defined. Meanwhile, corresponding guidelines for questionnaire investigators were formed.After review and approval of finalization, the process should have version management. The data management of this project will be run on tongji Brain Health integrated service platform developed by Xunfei.
