研究目的：

本课题通过研究超声引导下连续菱形肋间神经阻滞在胸科手术中的的应用，评估其在胸科手术中是否有良好的镇痛效果。  

Objectives of Study：

In this study, the application of ultrasonic guided continuous rhomboid intercostal nerve block in thoracic surgery was studied to evaluate whether it has good analgesic effect in thoracic surgery.

药物成份或治疗方案详述：

按照计算机生成的随机数字随机分为2组：对照组C组，试验组RIB组，每组 30 例。RIB组行连续菱形肌神经阻滞（0.2%罗哌卡因200ml），C组不进行任何神经阻滞，两组均行气管插管全身麻醉，术后C组均行48小时舒芬太尼PCIA，将舒芬太尼2ug/kg用0.9%氯化钠溶液稀释至100ml，无背景输注，追加剂量为2ml，锁定时间15分钟，RISS组行连续神经阻滞泵注（200ml，0.2%罗哌卡因，持续泵注4ml/h，追加剂量4ml，锁定时间15分钟），若术后484h内患者NRS评分>3分时，则按压镇痛泵1次。由同一组主刀医生负责手术，同一位麻醉医生负责进行麻醉，另一位麻醉医生负责访视评分及记录。 

Description for medicine or protocol of treatment in detail：

According to the random numbers generated by computer, they were randomly divided into two groups: control group C (n = 30) and experimental group RIB (n = 30). RIB group received continuous rhomboid muscle nerve block (0.2% ropivacaine 200ml), C group received no nerve block, and both groups received endotracheal intubation general anesthesia. 48 hours after operation, sufentanil PCIA, was performed in group C to dilute sufentanil 2ug/kg with 0.9% sodium chloride solution to 100ml without background infusion, additional dose was 2ml, locking time was 15 minutes, continuous nerve block pump was given in RISS group (200ml 0.2% ropivacaine). If the patient's NRS score was more than 3 within 484 hours after operation, the analgesia pump should be pressed once. The analgesia pump was injected continuously at 4ml / h, the additional dose was 4ml, and the locking time was 15 minutes. The same group of surgeons were responsible for the operation, the same anesthesiologist was responsible for anesthesia, and another anesthesiologist was responsible for visit scoring and recording. 

纳入标准：

2020年11月至2022年12月于嘉兴市第一医院行单侧胸腔镜肺叶切手术，患者60例，年龄18~80岁，ASA Ⅰ~Ⅲ级。

Inclusion criteria

From November 2020 to December 2022, 60 patients underwent unilateral thoracoscopic lobectomy in the first hospital of Jiaxing City, 60 patients, aged 18-80 years, ASA I - III.

排除标准：

(1)休克或昏迷状态；
(2)心、肺、肝、肾及凝血功能异常；
(3)拟行阻滞区域感染，阻滞侧肢体严重神经损伤；
(4)有慢性疼痛服用镇痛药物史、服用精神病物病史、放化疗病史，对非甾体类抗炎药物有禁忌症的患者；
(5)对局麻药如利多卡因、罗哌卡因以及全身麻醉药物过敏患者；
(6)患者及家属拒绝手术麻醉。

Exclusion criteria：

1. Patients in shock or coma;
2. Patients with abnormal heart, lung, liver, kidney and coagulation function;
3. For those patients with infection in the block area, severe nerve injury occurred in the block side;
4. Patients with chronic pain, history of taking analgesics, psychotics, radiotherapy and chemotherapy, and contraindications to non steroidal anti-inflammatory drugs;
5. Patients allergic to local anesthetics such as lidocaine, ropivacaine and general anesthetics;
6. Patients and their families refused anesthesia.
