研究目的：

（1）建立伏立康唑血药浓度与治疗安全性、有效性的关系，确定伏立康唑在重症患者人群的治疗窗。 （2）建立伏立康唑在重症患者中的肝毒性预测模型，避免临床因伏立康唑治疗而导致的肝损。 （3）建立的群体药动学模型，指导伏立康唑临床个体化用药，保证伏立康唑用药的安全性和有效性。  

Objectives of Study：

The aim of this study was to(1) establish the relationship between voriconazole plasma concentration and the safety and efficacy of voriconazole treatment, and determine the therapeutic window of voriconazole in critically ill patients,(2) establish a prediction model of voriconazole hepatotoxicity in critically ill patients to avoid clinical liver damage caused by voriconazole treatment,and (3)establish a population pharmacokinetic model of voriconazole and guide the clinical individualized medication of voriconazole to ensures the safety and efficacy of voriconazole therapy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）入住ICU，年龄≥18岁；
（2）确诊、临床诊断或疑诊为IFD及具有真菌感染高危因素患者；
（3）使用伏立康唑，用药时间≥7 d；
（4）对伏立康唑至少进行一次稳态（用药3天后）血药浓度监测；
（5）伏立康唑治疗期间，每周至少进行一次肝功检测。

Inclusion criteria

(1) patients were admitted to the ICU, and age ≥18 years old;
(2) Patients were diagnosed with a proven, probable, and possible invasive fungal disease, or with high risk factors for fungal infection;
(3) Using voriconazole for ≥7 d;
(4) Monitoring voriconazole blood concentration at least once at a steady state (3 days after administration);
(5) During voriconazole treatment, liver function tests should be performed at least once weekly.

排除标准：

（1）妊娠妇女、对伏立康唑过敏患者；
（2）患者血药浓度低于检测下限；
（3）患者服药时间不确定及无实验室检查项目结果的患者；
（4）根据CTCAE标准，基础肝功大于3级或以上者；
（5）入院时具有病毒性肝炎、肝硬化、药物性肝损、肝癌等严重肝脏疾病的患者。

Exclusion criteria：

(1) Pregnant women and patients are allergic to voriconazole;
(2) Voriconazole blood concentration is lower than the lower limit of detection;
(3) The time of administration of Medication is uncertain and laboratory test results are unavailable;
(4) According to CTCAE standards, patients with liver dysfunction is greater than grade 3 or above when admitted to ICU;
(5) Patients with severe liver diseases such as viral hepatitis, cirrhosis, drug-induced liver damage, liver cancer when admitted to ICU;
