研究目的：

1）基于基因技术沉默TGIF1基因的ASCs及其所分泌的外泌体治疗肌腱损伤的临床转化研究，通过分析肌腱损伤修复的现状及存在问题，探讨细胞工程技术可能应用于肌腱修复的发展方向，并对未来的实验室研究和临床应用进行指导和预测。 2）建立安全有效且具有自主知识产权、疗效显著的脂肪干细胞治疗技术，完成临床前研究并开展临床研究，建立操作规范，明确适应症、剂量、疗程等主要技术指标，建立对不良反应的监测、预防和治疗方案； 3）研发基因修饰的脂肪干细胞及其所分泌的外泌体规模化制备技术，建立操作规范和质量控制标准； 4）建立基因修饰的ASCs细胞及其所分泌的外泌体治疗肌腱损伤的安全性和有效性评估体系，进而能够显著提高肌腱疾病患者的康复率，亦能提升人民的生活质量。  

Objectives of Study：

1) Based on gene technology silencing tgif1 gene ASCs and their secreted exosomes for the treatment of tendon injury, analyze the current situation and existing problems of tendon repair, to explore the possible development direction of cell engineering technology in tendon repair, and to guide and predict the future laboratory research and clinical application 2) Establish a safe and effective adipose stem cell therapy technology with independent intellectual property rights and significant curative effect, complete preclinical research and carry out clinical research, establish operation specifications, define the main technical indicators such as indication, dosage, course of treatment, and establish the monitoring, prevention and treatment plan of adverse reactions; 3) To develop the large-scale preparation technology of gene-modified adipose stem cells and their secreted exosomes, and establish the operation specifications and quality control standards; 4) To establish the safety and effectiveness evaluation system of gene-modified ASCs cells and their secreted exosomes in the treatment of tendon injury, which can significantly improve the rehabilitation rate of patients with tendon diseases and improve the quality of life of the people.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄18-70周岁；②MRI/CT/超声确诊腱-骨止点撕裂或行肌腱修复手术的患者；③肌腱-骨止点撕裂、炎症，主要包括肩袖损伤、交叉韧带撕裂、跟腱炎、侧副韧带损伤、网球肘等疾病患者；④自愿签署知情同意书

Inclusion criteria

① Age: 18-70 years old; ② patients with tendon-bone insertion tear or tendon repair confirmed by MRI / CT / ultrasound; ③ patients with tendon-bone insertion tear and inflammation, mainly including rotator cuff injury, cruciate ligament tear, heel tendinitis, collateral ligament injury, tennis elbow, and other diseases; ④ voluntary signing of informed consent

排除标准：

①在最近的30天内接受过其他研究性药物的治疗；②同时在服用中药；③存在病毒活动性感染且研究负责人认为会降低患者耐受力；④怀孕期或哺乳期；⑤患者严重的并发症；⑥有严重心脏疾病，如充血性心衰病史、心律不齐、心绞痛等

Exclusion criteria：

① In the last 30 days, he has received other research drugs; ② he is taking traditional Chinese medicine at the same time; ③ there is a viral infection and also the research leader thinks it will reduce the patient's tolerance; ④ pregnancy or lactation; ⑤ severe complications of the patient; ⑥ severe heart disease, such as the history of congestive heart failure, arrhythmia, angina pectoris, etc
