使用经颅磁电刺激干预轻度认知障碍患者，分析对社区及门诊的治疗有效性与可及性。  

Objectives of Study：

To analyze the effectiveness and accessibility of transcranial magnetic stimulation to patients with mild cognitive impairment in community and outpatient Settings.

药物成份或治疗方案详述：

经颅磁电刺激技术干预：针对50例来源于上海市精神卫生中心门诊、以及社区筛查的轻度或中度认知功能障碍患者，分为刺激组和对照组，治疗4周进行观察。以神经心理学评估、神经精神评估（NPI）作为终点指标。 

Description for medicine or protocol of treatment in detail：

Transcranial magnetoelectric stimulation technology intervention: 50 patients with mild or moderate cognitive impairment from the outpatient department of Shanghai Mental Health Center and community screening are divided into stimulation group and control group, and the treatment lasted for 4 weeks. Neuropsychological assessment and neuropsychiatric assessment (NPI) are used as end points. 

纳入标准：

(1)社区老人或门诊患者；
（2）符合上述认知障碍诊断标准（可疑痴呆、轻度痴呆和中度痴呆）包括：CDR评分0.5-2；MMSE≤26分；
（3）年龄≥60岁，男女不限；
（4）小学或5年以上受教育程度；
（5）患者本人或其法律监护人签署知情同意书。

Inclusion criteria

1. Community elderly or outpatients;
2. The patients (suspected dementia, mild dementia and moderate dementia) who met the above diagnostic criteria for cognitive impairment included: CDR score 0.5-2; MMSE ≤ 26 points; MMSE ≤ 26 points;
3. The age of patients over 60 years old is not limited to men and women;
4. Patients with primary school or more than 5 years of education;
5. Informed consent signed by the patient or his legal guardian.

排除标准：

（1）排除有严重躯体疾病患者，自杀企图者；
（2）近一月内脑卒中、脑血管意外，癫痫病史，颅脑外伤病史；
（3）经SCID访谈排除痴呆以外的其它轴I精神障碍，包括：精神分裂症、情感性精神障碍、焦虑谱系障碍；
（4）排除视听功能差或躯体疾病不能完成认知与情绪评估者；
（5）研究者认为存在不适合参加该临床试验的其他情况。

Exclusion criteria：

1. Patients with serious physical diseases and suicide attempts were excluded;
2. Patients with history of stroke, cerebrovascular accident, epilepsy and craniocerebral trauma in recent one month;
3. Those patients who excluded other Axis I mental disorders, including schizophrenia, affective disorder and anxiety spectrum disorder, were interviewed by SCID;
4. The patients with poor audiovisual function or physical diseases who could not complete the cognitive and emotional assessment were excluded;
5. The researcher thinks that there are other situations that are not suitable for participating in the clinical trial.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.线下评估病例报告表（CRF）由本课题组指定专人统一整理并保管，仅在数据处理人员录入数据、核对数据时将病例报告取出，完成上述工作后及时原样放回，原始数据保存10年。 2.电子采集和系统管理认知障碍模块，设立计算机电子管理员，提醒跟踪随访提醒并更新。在线下设立人工病案管理员，应答电子管理的跟踪提醒，在分级诊疗与预警处理时予以对应资源转介与联络。受试者的编码、个人信息等敏感信息由病案管理员保管，存放于加密电子文档内，并进行备份，保存10年。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. The offline evaluation case report Form (CRF) shall be organized and kept by the designated person of our research group. The case report shall be taken out only when data processing personnel input data and check data. 2. Electronic acquisition and system management module of cognitive impairment; set up computer electronic administrator to remind, track, follow up and update. The artificial medical record manager shall be set up offline to respond to the tracking reminder of electronic management, and to refer and contact corresponding resources during hierarchical diagnosis and early warning processing. The subject's code, personal information and other sensitive information shall be kept by the medical record administrator and stored in the encrypted electronic document, which shall be backed up and stored for 10 years.
