研究目的：

选择帕金森病早、 中、 晚期患者， 采用随机对照试验， 验证课题一制定的老年常见功能障碍的康复体系-老年帕金森病康复管理体系的有效性和安全性， 探讨三级医院、 康复科/康复医院、 社区、 家庭康复过程中， 帕金森病全周期临床-康复-护理衔接模式及多学科合作模式，规范帕金森病患者全周期管理策略。  

Objectives of Study：

To verify the effectiveness and safety of the rehabilitation management system for Parkinson's disease in the elderly, which is a rehabilitation system for the common dysfunction of the elderly developed in topic 1, we selected patients with Parkinson's disease in the early, middle and late stages, and used randomized controlled trials to explore the process of rehabilitation in tertiary hospitals, rehabilitation / rehabilitation hospitals, communities and families, Objective to standardize the management strategy of Parkinson's disease patients in the whole cycle.

纳入标准：

1. 年龄≥65 岁；
2. 符合 2015 年国际运动障碍协会（Movemrnt Disorder Society,） 帕金森综
合征诊断标准；
3. 预计生存年限≥2 年；
4. 稳定服用抗帕金森药物者；
5. 患者或家属签署知情同意书。

Inclusion criteria

1. Patients over 65 years old;
2. In accordance with the 2015 International Association for movement disorders (movemrnt disorder society,) Parkinson's syndrome
Patients who met the diagnostic criteria;
3. Patients with an estimated survival time of more than 2 years;
4. Patients taking anti Parkinson drugs stably;
5. Informed consent signed by patients or their families.

排除标准：

1.其他锥体外系疾病或诊断不明确者；
2.头 CT/MRI 提示存在颅内占位性病变者；
3.合并心脏、 肝脏、 肾脏、 血液系统、 自身免疫系统等其他严重脏器或系统
性疾病；
4.合并其他原因导致的步态姿势异常症状者， 如骨关节疾病等；
5.正在参与其他干预性研究者；
6.严重认知障碍、 焦虑抑郁及其他无法配合康复评定与康复训练者。

Exclusion criteria：

1. Patients with other extrapyramidal diseases or unclear diagnosis;
2. Patients with intracranial space occupying lesions indicated by CT / MRI;
3. Complicated with heart, liver, kidney, blood system, autoimmune system and other serious organs or systems
Patients with sexual diseases;
4. Patients with abnormal gait and posture caused by other reasons, such as bone and joint diseases;
5. Patients who are participating in other intervention studies;
6. Severe cognitive impairment, anxiety and depression and other patients who can not cooperate with rehabilitation assessment and rehabilitation training.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：
