研究目的：

本课题旨在观察热痹膏对活动性湿热痹阻证类风湿关节炎患者的DAS28评分、中医证候积分量表、患者结局临床报告、关节功能分级、外用药局部不良反应评价量表等的影响，初步探讨热痹膏对类风湿关节炎（湿热痹阻证）的关节局部症状、体征的改善作用以及不良反应，为今后深入研究和临床推广热痹膏奠定基础，为临床上中药膏剂治疗类风湿关节炎提供新的药物。  

Objectives of Study：

This topic aims to observe the DAS28 score, TCM syndrome score scale, clinical report of patient outcome, joint function classification, local adverse reaction evaluation scale of topical drugs, etc. of Rebi ointment on patients with active rheumatoid arthritis with damp-heat arthralgia syndrome. To discuss the effect of Rebi ointment on the joint local symptoms and signs of rheumatoid arthritis (damp-heat block syndrome) and its adverse reactions, which will lay the foundation for in-depth research and clinical promotion of heat bi ointment in the future, and will be used for clinical treatment of traditional Chinese medicine ointment. Rheumatoid arthritis provides new drugs.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）符合2010年ACR/EULAR的RA诊断标准；
（2）符合尪痹-湿热痹阻证中医证型诊断标准；
（3）年龄在18-70岁之间，性别不限；
（4）双手关节X线分期属Ⅰ、Ⅱ期，关节功能分级属Ⅰ、Ⅱ、Ⅲ级；
（5）2.6分＜DAS28评分＜5.1分；
（6）至少存在双腕关节疼痛或肿胀；
（7）使用激素、激素疑似物或生物制剂患者需要停药3个月以上；
（8）入组前至少1周未使用非甾体类抗炎或外用镇痛药。
（9）自愿受试，签署知情同意书。

Inclusion criteria

(1) Meet the 2010 ACR/EULAR RA diagnostic criteria;
(2) Comply with the diagnostic criteria of TCM syndrome type of 尪bi-damp-heat resistance syndrome;
(3) Between 18-70 years old, no gender limit;
(4) The X-ray staging of the joints of both hands belongs to stage I and stage II, and the classification of joint function belongs to stage I, II, and III;
(5) 2.6 points (6) At least there is pain or swelling in both wrist joints;
(7) Patients who use hormones, hormone suspects or biological agents need to stop the drug for more than 3 months;
(8) No non-steroidal anti-inflammatory or topical analgesics were used for at least 1 week before enrollment.
(9) Take the test voluntarily and sign the informed consent form.

排除标准：

（1）合并其他风湿免疫性疾病者，如系统性红斑狼疮、干燥综合征、严重骨关节炎等；
（2）合并心血管、肺部、肝脏、胃肠道、肾脏、造血系统或肿瘤等严重疾病，或合并各种急慢性感染或其他传染性疾病（肝炎、结核等）；
（3）受试关节局部皮肤破损、感染或外伤未愈者；
（4）对甲氨蝶呤、来氟米特治疗无反应者；
（5）有试验药物禁忌症（肺间质纤维化等）、过敏史；
（6）有酗酒、吸毒史或神经精神异常病史（癫痫、抑郁症等）；
（7）妊娠或哺乳期妇女，近期有生育计划；
（8）正在参与其它药物临床研究。

Exclusion criteria：

(1) Patients with other rheumatic immune diseases, such as systemic lupus erythematosus, Sjogren’s syndrome, severe osteoarthritis, etc.;
(2) Combined with serious diseases such as cardiovascular, lung, liver, gastrointestinal tract, kidney, hematopoietic system or tumor, or combined with various acute and chronic infections or other infectious diseases (hepatitis, tuberculosis, etc.);
(3) Partial skin of the tested joint is damaged, infected or unhealed;
(4) Those who do not respond to methotrexate or leflunomide treatment;
(5) Contraindications of test drugs (pulmonary interstitial fibrosis, etc.), history of allergies;
(6) A history of alcoholism, drug abuse, or neuropsychiatric disorders (epilepsy, depression, etc.);
(7) Pregnant or breast-feeding women, have a childbirth plan in the near future;
(8) Participating in clinical research of other drugs.
