研究目的：

（1）基于Proms和综合全球证据，聚焦COPD患者疾病全周期的自我管理，构建COPD患者自我管理干预方案。 （2）构建具有人工智能辅助决策功能的COPD自我管理平台。 （3）开展实验性研究，验证COPD患者自我管理模式的可行性和有效性，以期制订慢阻肺的自我护理管理推荐。  

Objectives of Study：

1. Based on proms and comprehensive global evidence, focus on the self-management of COPD patients in the whole disease cycle, and construct self-management intervention program for COPD patients. 2. Construct COPD self-management platform with the function of artificial intelligence aided decision-making. 3. Carry out experimental research to verify the feasibility and effectiveness of self-management model for COPD patients, so as to formulate self-care management recommendations for COPD patients.

药物成份或治疗方案详述：

纳入标准：

①诊断符合2019 GOLD
②COPD稳定期，生活基本能够自理
③可以接受电话随访
④配备智能化手机并愿意使用
⑤研究对象具有基本的理解和阅读能力，无精神障碍类疾病，且已签订知情同意书。

Inclusion criteria

1. Diagnosis of patients with gold in 2019;
2. Patients with stable COPD can basically take care of themselves;
3. Patients who can accept telephone follow-up;
4. Patients who are equipped with smart phones and willing to use them;
5. Patients with basic understanding and reading ability, no mental disorders, and have signed informed consent.

排除标准：

①曾行肺减容术、肺移植术或肺切除术
②肢体功能活动障碍
③认知功能障碍
④家住外省无法随访。

Exclusion criteria：

1. Patients who have undergone lung volume reduction, lung transplantation or pneumonectomy;
2. Patients with limb dysfunction;
3. Patients with cognitive impairment;
4. Patients living in other provinces who cannot be followed up.
