研究目的：

拟通过比较不同剂量艾司氯胺酮在腹腔镜结直肠癌根治术老年患者术后镇痛的效果，观察艾司氯胺酮能否减少阿片类药物的用量和痛觉过敏，探讨其围术期镇痛效应。通过观察患者术后炎症因子的变化、比较患者术后认知功能障碍的发生率，探讨艾司氯胺酮对老年患者术后认知功能的影响。  

Objectives of Study：

It is planned to compare the analgesic effects of different doses of esketamine in elderly patients undergoing laparoscopic radical resection of colorectal cancer to observe whether esketamine can reduce the dosage of opioids and hyperalgesia, and to explore its perioperative analgesic effect. By observing the changes of postoperative inflammatory factors and comparing the incidence of postoperative cognitive dysfunction in patients, the effect of esketamine on postoperative cognitive function of elderly patients was explored.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 行择期腹腔镜结直肠癌根治术的老年患者；
2. 年龄>60岁；
3. ASA分级：II-III级；
4. 患者及委托人同意参加本研究，并签署知情同意书。

Inclusion criteria

1. Elderly patients undergoing elective laparoscopic radical resection for colorectal cancer;
2. Patients over 60 years old;
3. ASA grade: II-III;
4. Patients and clients agreed to participate in the study and signed informed consent.

排除标准：

1. 既往有神经及精神病史；
2. 正在使用其他阿片类药物治疗急慢性疼痛、目前服用相关的抗神经及抗精神药物史；
3. 存在严重的心、脑、肝、肾等疾病史；
4. 酗酒或阿片成瘾；
5. 存在严重的听觉、视觉障碍，文盲、有沟通障碍及依从性差者等。行手术老年患者的年龄、性别、体重、受教育程度、手术时间、麻醉时间、术中失血量、术中阿托品及麻黄碱等被纳入。

Exclusion criteria：

1. Patients with previous neurological and psychiatric history;
2. Patients who are using other opioids to treat acute and chronic pain and who have been taking relevant antineural and antipsychotic drugs;
3. Patients with serious history of heart, brain, liver and kidney diseases;
4. Alcoholism or opioid addiction;
5. Patients with severe hearing and visual impairment, illiteracy, communication barriers and poor compliance. The age, gender, weight, education level, operation time, anesthesia time, intraoperative blood loss, atropine and ephedrine were included.
