研究目的：

生物資料庫的建立為目前臨床醫學與預防醫學急需完成的一項任務，這些資料可以用來提供臨床診斷與鑑定環境暴露與風險評估的生物指標。利用蛋白質體的分析技術來探討可能的小分子生物指標能應用於臨床診斷的協助以及環境暴露所造成的不良健康效應。流行病學研究指出環境中的暴露，例如:抽菸與空氣汙染，會引起肺癌與心臟衰竭的發生率增加以及增加肺結核的感染率。臨床上這些疾病的發生，往往會引起一連串嚴重的發炎反應而導致肋膜積水的情況。過去研究嘗試以肋膜積水的生物檢體探討肺癌發生可能的生物指標，以期達到公共衛生早期診斷的觀念，但是這些病患在過去的環境暴露狀況以及疾病的發生之間相關性甚少討論。本研究希望藉由蛋白質體的分析技術以探討環境(抽菸與空氣汙染)和心肺疾病(肺癌、心臟衰竭、肺結核與肺炎)之間的相關性，藉由本研究結果找出可能的生物指標在肺癌、心臟衰竭、肺結核與肺炎上以提供環境暴露風險評估與臨床診斷使用。  

Objectives of Study：

The purpose of this study is to identify biomarkers in patients with (1) lung cancer, (2) heart failure, (3) TB and (4) pneumonia, for diagnostic use and to evaluate the influence of environmental exposure. Figure 1 briefly shows the design of this study. To achieve our objectives, a 4-year research plan will be conducted patients with lung cancer (Year 1), heart failure (Year 2) and TB (Year 3) and pneumonia (Year 4). Pleural effusions will be collected from patients. The patient’s history of exposure to tobacco smoke or data from appropiate air pollution monitor stations will be obtained. In addition, a patient’s medication history will be incorporated into the analysis of changes in biomarkers in their samples. Furthermore, in vivo study will be performed to examine the role of the identifying protein markers. The animal results can provide better understanding of the biological pathway regarding the protein and confirmatory results.

肺癌:
1. 受試者為組織學診斷為惡性腫瘤者(包括小细胞肺癌與非小細胞肺癌)
2. 年齡需大於20歲並簽署受試者同意書
3. 受試者因胸膜腔渗液累積造成呼吸困難(此為必要之臨床診斷與檢驗，不會因
研究所需而取得額外的檢體或檢查)
心臟衰竭:
1. 受試者為臨床診斷確認者
2. 年齡需大於20歲並簽署受試者同意書
3. 受試者因胸膜腔渗液累積造成呼吸困難(此為必要之臨床診斷與檢驗，不會因研究所需而取得額外的檢體或檢查)
肺結核與肺炎
1. 受試者為細菌培養確認者(肺結核部分包括肺內與肺外結核) 2. 年齡需大於20歲並簽署受試者同意書 3. 受試者因胸膜腔渗液累積造成呼吸困難(此為必要之臨床診斷與檢驗，不會因研究所需而取得額外的檢體或檢查)

Inclusion criteria

Study populationParticipants with lung cancer (n = 60), heart failure (n = 30), TB (n = 60) or pneumonia (n = 30) will be recruited from the Shuang Ho Hospital (Taipei Medical University). Patient characteristics and histories, including age, sex and previous hospital admissions, will be collected for each participant. All personal ID will be masked and encoding.The study criteria are as follows:Lung cancera)Patients with a histologically proven malignancy (including SCLC and NSCLC)b)Patients who are more than 20 years old and have provided signed informed consentc)Patients in whom the indication for pleural effusion thoracentesis was exertional dyspnoea (no additional samples will be collected and no unnecessary surgical procedures will be performed)Exclusion criteria:a)HIV (+) patientsHeart failureInclusion criteria:a)Patients with a confirmed clinical diagnosisb)Patients who are more than 20 years old and have provided signed informed consentc)Patients in whom the indication for pleural effusion thoracentesis was exertional dyspnoea (no additional samples will be collected and no unnecessary surgical procedures will be performed)Exclusion criteria:a)HIV (+)TB and pneumoniaInclusion criteria:a)Patients with culture-proven TB, including pulmonary and extrapulmonary TB and pneumoniab)Patients who are more than 20 years old and have provided signed informed consentc)Patients in whom the indication for pleural effusion thoracentesis was exertional dyspnoea (no additional samples will be collected and no unnecessary surgical procedures will be performed)
