研究目的：

（1）基于Proms和综合全球证据，聚焦COPD患者疾病全周期的自我管理，构建COPD患者自我管理干预方案。 （2）构建具有人工智能辅助决策功能的COPD自我管理平台。 （3）开展实验性研究，验证COPD患者自我管理模式的可行性和有效性，以期制订慢阻肺的自我护理管理推荐。  

Objectives of Study：

(1) Based on Proms and comprehensive global evidence, focus on the self-management of COPD patients throughout the disease cycle, and construct a self-management intervention plan for COPD patients. (2) Building a COPD self-management platform with AI-assisted decision-making functions. (3) Carring out experimental studies to verify the feasibility and effectiveness of the self-management model for COPD patients, with a view to formulating COPD self-care management recommendations.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①诊断符合2019 GOLD ②COPD稳定期，生活基本能够自理 ③可以接受电话随访 ④配备智能化手机并愿意使用 ⑤研究对象具有基本的理解和阅读能力，无精神障碍类疾病，且已签订知情同意书。

Inclusion criteria

①Diagnosis is in compliance with 2019 GOLD ②COPD is stable and able to take care of life basically ③Can accept telephone follow-up ④Equipped with smart phone and willing to use ⑤Research subjects have basic understanding and reading ability, no mental disorders, and have signed informed consent.

排除标准：

①曾行肺减容术、肺移植术或肺切除术 ②肢体功能活动障碍 ③认知功能障碍 ④家住外省无法随访。

Exclusion criteria：

①Pulmonary volume reduction surgery, lung transplantation or pneumonectomy has been performed ②Limb dysfunction ③Cognitive dysfunction ④Family living in other provinces cannot be followed up.
