研究目的：

评价左心耳封堵器系统用于长期口服抗凝治疗禁忌或抗凝治疗后仍有卒中风险的非瓣膜性心房颤动患者进行左心耳封堵治疗的有效性和安全性。  

Objectives of Study：

Evaluating the efficacy and safety of left atrial appendage system for left atrial appendage occlusion in patients with non-valvular atrial fibrillation, who are still at risk of stroke after long-term oral anticoagulation or contraindication of anticoagulation therapy.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）CHA2DS2-VASc评分≥2 分的非瓣膜性心房颤动患者；
（2）符合以下情况之一：
①长期口服抗凝治疗禁忌的患者。
②服用华法林、国际标准化比值（INR)达标的基础上仍发生脑卒中或血栓栓塞事件的患者。
③HAS-BLED评分≥3分者。
④不愿/无法耐受长期口服抗凝治疗的患者。
(3)年龄≥18岁；
(4)自愿参加本次临床试验，并已签署知情同意书。

Inclusion criteria

1. CHA2DS2-VASc scores >= 2 in patients with non-valvular atrial fibrillation;
2. It meets one of the following circumstance:
(1) The patients who are contraindication of long-term oral anticoagulation therapy.
(2) Patients with stroke or thromboembolism still occur on the basis of warfarin and international standardized ratio (INR).
(3) HAS-BLED scores >= 3.
(4) Patients unwilling/unable to tolerate long-term oral anticoagulation therapy.
3. Aged >= 18 years.
4. Voluntary participation in this clinical trial and informed consent signed.

排除标准：

(1) 偶发的心房颤动。
(2) 对镍、华法林、肝素、阿司匹林、氯吡格雷过敏或服用华法林、肝素、阿司匹林、氯吡格雷期间出现严重并发症。
(3) 近3个月内有心肌梗死或冠脉影像学明确存在严重且需要干预的冠脉病变。
(4) 近30天内新发的卒中。
(5) 左心耳封堵器植入30天内有行射频消融术，或有电复律计划。
(6) 曾行左心耳切除术（无明显残余腔）、人工心脏机械瓣膜置换术、房间隔缺损封堵器植入术、心脏移植术等心脏相关手术且不再适合开展左心耳封堵手术的患者。
(7) 合并出血性疾病、未痊愈的消化道溃疡。
(8) 合并静脉血栓栓塞，包括不限于脑、肝、肾、肠系膜静脉血栓。
(9) 合并任何类型的活动性感染。
(10) 合并需华法林抗凝治疗的除房颤外的其他疾病。
(11) 合并各系统其它严重疾病的患者，包括不限于严重低血压、休克、肺功能不全、恶性肿瘤。
(12) 血小板减少症(﹤6万/mm3)的患者。
(13) 心房前后径>65mm、经食管超声心动图（TEE)发现心内血栓和/或左心耳浓密自发显影、心包积液>3mm 、心脏肿瘤、心内有赘生物。
(14) 超声心动图测量左心耳开口直径<10.5mm或>35mm , 或深度<10mm。
(15) 左室射血分数(LVEF)<0.35。
(16) 心功能III级或IV级（NYHA分级）且暂未纠正者。
(17) 预计生存期<1年的患者。
(18) 妊娠或在本试验进行期间有计划怀孕者。
(19) 入选前1月内参加过其它临床试验的患者。
(20) 研宄者认为不宜参加本临床试验的患者。

Exclusion criteria：

(1) Incidental atrial fibrillation
(2) There are contraindications to the use of warfarin, aspirin, or clopidogrel.
(3) There were serious myocardial infarction or serious coronary artery imaging in the past 3 months.
(4) Stroke in recent 30 days.
(5) There is a plan of radiofrequency ablation or electrical cardioversion will be performed within 30 days after the implantation of left atrial appendage occluder.
(6) Patients who have undergone heart-related operations such as left atrial appendectomy (no obvious residual cavity), mechanical heart valve replacement, atrial septal defect occluder implantation, heart transplantation, etc., are no longer suitable for left atrial appendage occlusion.
(7) Complicated with hemorrhagic diseases and unhealed peptic ulcer.
(8) Complicated with venous thromboembolism, including not limited to cerebral, liver, kidney, mesenteric venous thrombosis.
(9) Merge any type of active infection.
(10) Combined with other diseases besides atrial fibrillation requiring warfarin anticoagulation therapy.
(11) Patients with other serious diseases of various systems, including not limited to severe hypotension, shock, pulmonary insufficiency, malignant tumors.
(12) Patients with thrombocytopenia (﹤60,000/mm3).
(13) Atrial anteroposterior diameter >65mm, transesophageal echocardiography (TEE) revealed intracardiac thrombosis and/or left atrial dense spontaneous development, pericardial effusion > 3mm, cardiac tumors, and intracardiac neoplasms.
(14) Left atrial appendage diameter <10.5 mm or >35 mm or depth <10mm were measured by TEE.
(15) Left ventricular ejection fraction (LVEF) <0.35.
(16) Grade III or IV of cardiac function (NYHA) without correction.
(17) Patients with a predicted survival of less than 1 year.
(18) Pregnancies or planned pregnancies during this trial.
(19) Patients who had participated in other clinical trials within one month before admission were enrolled.
(20) Researchers believe that patients who are not eligible to participate in this clinical trial are not eligible.
