研究目的：

本研究计划拟将“均衡营养素膳食”推广应用到140家妇幼保健机构或二级医院，对入选机构医务人员进行系统培训，提高基层诊疗水平，纳入5000例患者， 探索其普遍性、一般性规律，寻找适合地区人群的重要治疗切点，研发功能性食品；进一步优化技术指南及临床路径，最终形成电子化体系，联合技术单位开发完善电子化平台；为国家生育、健康政策的制定提供数据支持。  

Objectives of Study：

This study plans to extend the "balanced nutrient diet" to 140 maternal and child health care institutions or secondary hospitals, to systematically train medical personnel in selected institutions, to improve the level of diagnosis and treatment at the grass-roots level, to include 5000 patients, to explore its universality and general laws, to find important treatment points suitable for the local population, to develop functional foods, to further optimize technical guidelines and clinical pathways, to form an electronic system, to develop and improve electronic platforms in conjunction with technical units, and to provide data support for the formulation of national fertility and health policies.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

患有多囊卵巢综合征且超重或者肥胖的患者
1. 多囊卵巢综合征
1）符合《多囊卵巢综合征中国诊疗指南(2018》诊断标准的患者；
2）此前未应用二甲双胍治疗PCOS的患者；
3）此前未应用二甲双胍治疗糖尿病等其他疾病的患者；
2.超重和肥胖的患者
1）根据BMI（Body Mass Index，简称BMI）评价患者的标准体重，BMI是用体重千克数除以身高米数的平方得出的数字，是国际上常用的衡量人体胖瘦程度以及是否健康的一个标准。
2）根据中国参考标准，BMI≥24被认为是超重，BMI≥28被认为是肥胖；

Inclusion criteria

Patients with polycystic ovary syndrome and overweight or obese
1. polycystic ovary syndrome
1) patients who meet the Chinese diagnostic criteria of PCOS (2018);
2) patients who had not previously treated PCOS with metformin;
3) patients who had not previously treated other diseases such as diabetes with metformin;
2. overweight and obese patients
The standard weight of a patient 1) evaluated on the basis of BMI (Body Mass Index), BMI is the number of kilograms divided by the square of the number of meters in height, which is a commonly used international measure of weight and health.
2) BMI≥24 is considered overweight and BMI≥28 is considered obese according to the Chinese reference standard;

排除标准：

1）有其他严重疾病的患者，包括冠心病（心绞痛、心肌梗死、冠状动脉血管再生过程或心电图异常Q波出现(ECG)）、中风（缺血性或出血性，包括短暂性脑缺血发作）等；
2）有营养障碍性疾病或会影响营养干预效果的严重胃肠道疾病的患者；
3）任何其他医疗条件认为最长生存时间低于5年的患者;
4）有药物滥用或慢性酗酒或酒精依赖等成瘾性倾向的患者；
5）对改变饮食习惯或遵照医嘱配合改善生活习惯有很大执行上的困难的患者或不能坚持随访的患者;
6）对二甲双胍过敏或者既往应用二甲双胍出现严重副反应不能耐受的患者。

Exclusion criteria：

Patients with 1) other serious diseases include coronary heart disease (angina pectoris, myocardial infarction, coronary artery regeneration process or abnormal ECG Q (ECG)), stroke (ischemic or hemorrhagic, including transient ischemic attack), etc.;
2) patients with severe gastrointestinal diseases with nutritional disorders or which may affect the effects of nutritional interventions;
3) any other medical condition that the maximum duration of survival is less than 5 years;
4) patients with addictive sexual tendencies such as substance abuse or chronic alcohol or alcohol dependence;
5) patients who have significant implementation difficulties in changing their eating habits or following doctor's orders to improve their living habits or who are unable to follow up;
6) allergy to metformin or severe side effects of previous use of metformin are intolerable.
