选择 AD 疾病分期中： 轻度认知损害期和痴呆期的患者， 采用随机对照研究， 探索课题一制定的—老年常见功能障碍的康复体系-阿尔茨海默病功能障碍康复体系的有效性和安全性， 探讨三级医院综合医院神经内科、 康复科/康复医院、 社区、 家庭康复过程中， 阿尔茨海默病全周期临床-康复-护理衔接模式及多学科合作模式， 规范阿尔茨海默病患者全周期管理策略。  

Objectives of Study：

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1） 年龄≥65周岁；
（2） 符合2011年美国国立老化研究所和阿尔茨海默病协会(Nalional Institute on Aging-Alzheimer‘s Assoeiation， NIA-AA)提出的AD诊断标准；
（3） 预计生存年限大于等于2年；
（4） 患者或家属签署知情同意书。

Inclusion criteria

(1) Age ≥ 65 years old;
(2) Meet the AD diagnostic criteria proposed by the National Institute on Aging-Alzheimer’s Assoeiation (NIA-AA) in 2011;
(3) The expected life span is greater than or equal to 2 years;
(4) The patient or family members signed an informed consent form.

排除标准：

（1） 诊断不明确患者；
（2） 头部 CT/MRI 提示颅内器质性病变者
（3） 严重失语或认知功能障碍或精神疾患等原因导致无法理解研究者指令， 无
法配合研究的受试者；
（4） 其他疾病引起的认知功能障碍；
（5） 正在参与其他干预性研究；
（6） 合并心脏、 肺、 肝脏、 肾脏、 内分泌、 血液系统严重疾病
（7） 无法配合康复治疗和评估的患者。

Exclusion criteria：

(1) Patients with unclear diagnosis;
(2) Those with head CT/MRI suggesting intracranial organic disease
(3) Severe aphasia, cognitive dysfunction or mental illness, etc., which lead to the inability to understand the instructions of the investigator, none
