研究目的：

通过制定个性化的延续性护理方案，改善患者的生活质量和免疫功能，提高护理疗效和护理满意度，为血液透析患者居家护理的研究提供参考  

Objectives of Study：

Objective to improve the quality of life and immune function of patients, improve the nursing effect and nursing satisfaction, and provide reference for the study of home care for hemodialysis patients

药物成份或治疗方案详述：

纳入标准：

①有明确的糖尿病病史；②临床表现及辅助检查结果证实患者为糖尿病肾病终末期；③肾功能不全由糖尿病引起，排除其他病因；④接受维持性血液透析治疗。⑤年龄在≥65岁。

Inclusion criteria

① There was a clear history of diabetes mellitus; ② the clinical manifestations and auxiliary examination results confirmed that the patient was the end-stage of diabetic nephropathy; ③ renal insufficiency was caused by diabetes, and other causes were excluded; ④ patients received maintenance hemodialysis treatment. ⑤ The age was 65 years or above.

排除标准：

①入院时即伴有其他系统严重病变；②合并急，慢性感染、恶性肿瘤等严重并发症；③有精神疾病或思维不清、痴呆的患者；④ 已参与其他类似研究的患者；⑤拒绝和无法完成本研究的患者。

Exclusion criteria：

① They were accompanied with other serious diseases when they were admitted to hospital; ② patients with acute and chronic infection, malignant tumor and other serious complications; ③ patients with mental illness or unclear thinking and dementia; ④ patients who have participated in other similar studies; and ⑤ patients who refuse or cannot complete this study.

干预措施：

①构建延续性护理单元。由科室主任、护士长、临床营养师、2名本科以上学历的血液透析科医生和病区护理人员组成。②建立患者个人健康档案。了解患者生活饮食习惯、文化水平、疾病历史、家庭经济状况、依从性以及自身的疾病了解程度、心理状态等方面。③制定延续性护理方案。根据患者的具体情况为患者制定个性化的居家护理方案，内容包括饮食计划、用药护理、生活方式的改变和面临突发状况的简单急救措施等。抽出每周一天透析日上机前，对患者及家属进行护理效果的评估，及时了解患者所面临的问题并给予相应的解决；每月由研究组业务骨干进行上门随访，观察方案实施情况，分析影响因素并调整方案计划。
