研究目的：

评估强化多危险因素干预联合心肌缺血诊断对并发颈动脉粥样硬化的2型糖尿病患者心血管终点事件发生率的影响，比较颈动脉粥样硬化和心肌核素显像在并发颈动脉粥样硬化的2型糖尿病患者心血管疾病的诊断和预测能力  

Objectives of Study：

To evaluate the effect of intensified multifactorial intervention combined with earlily detection of myocardial ischemia on the rates of main end point events in patients with type 2 diabetes and carotid artery atherosclerosis. To compare the diagnostic and prognostic value of carotid artery intima-media thickness(CIMT) and myocardial perfusion imaging on main end point events in type 2 diabetes with carotid artery atherosclerosis.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.年龄45～80岁之间，性别不限；
2.按照WHO1999年糖尿病诊断标准确诊的2型糖尿病患者。进行糖尿病治疗稳定3个月
3.颈动脉内-中膜厚度超过1mm或出现斑块（斑块直径大于1 mm）
4.有能力完成本研究进行的血糖和危险因素管理和接受心功能检查并签署知情同意书者

Inclusion criteria

1.Male and female aged 45 to 80 years old
2.Diagnosis of type 2 diabetes mellitus according to the WHO(1999) criteria and stable diabetes therapy for at 3 months
3. CIMT >=1 mm or exist plaque(diameter>=1mm)
4.Ability to perform all tasks related to glycemic control and risk-factor management and accepted detection of myocardial function and informed written consent

排除标准：

1）肝肾功能不良者（血肌酐超过正常上限1.2倍，ALT超过正常上限2倍）；
2）在半年以内合并酮症或酮症酸中毒、高渗昏迷、严重应激状态者；
3）怀孕或辅乳期妇女
4）患者有精神病史；
5）出血倾向；
6）非心脏病性致命性疾病；
7）酗酒或滥用药物史；
8）过去4周曾参加其他研究者；
9）因地域问题不能完成定期随访者；
10）不签署知情同意书者

Exclusion criteria：

1.Hepatic and renal disease (serum creatinine >=1.2 times the upper limit of normal,alanine aminotransferase >=2 times the upper limit of normal)
2.Existed ketosis or etoacidosis or hyperosmolar nonketotic diabetic coma or gross stress reaction in the past half a year
3.Pregnant or lactating women
4.History of mental aberration
5.Hemorrhagic tendency
6.Noncardiac illness expected to limit survival
7.Current alcohol abuse
8.Enrolled in a competing randomized trial or clinical study in the past 4weeks
9.Geographically inaccessible or unable to return for follow-up
10.Unable to understand or cooperate with protocol requirements
