研究目的：

探讨腔镜甲状腺近全切除术序贯131碘和甲状腺素抑制治疗低危分化型甲状腺癌的适应症，提出并制订腔镜手术操作规范，明确腔镜手术的常见并发症及预防措施，评估腔镜甲状腺近全切除术序贯131碘和甲状腺素抑制治疗分化型甲状腺癌的近远期疗效，为分化型甲状腺癌的治疗探索微创、有效、合理的新方案。  

Objectives of Study：

To explore the indications, surgical practices, complications and preventive measures of endoscopic near-total thyroidectomy, and to evaluate the clinical outcomes of endoscopic near-total thyroidectomy plus radioiodine ablation and TSH suppression therapy for the treatment of low-risk differentiated thyroid carcinoma.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

①年龄不大于60岁；②肿瘤直径小于4cm；③术前彩超示双侧颈部未发现明显肿大或异常淋巴结（直径>1cm），排除近期内曾患颈部或口咽部炎症等疾患；④术前影像学检查或术中未发现肿瘤浸出甲状腺包膜，⑤术前经细针穿刺活检或腔镜术中冰冻切片明确诊断为分化型甲状腺癌；⑥患者知情同意进行腔镜手术或中转进行甲状腺近全切加功能性颈淋巴结清扫。

Inclusion criteria

①Age=<60 years;
②the largest tumor diameter smaller than 4 cm;
③after excluding the effect of cervical and oropharyngeal inflammation, no obviously enlarged or abnormal cervical lymph nodes (>1 cm) found with preoperative color ultrasonography;
④no tumor invasion of thyroid capsule found preoperatively and intraoperatively;
⑤differentiated thyroid carcinoma confirmed histopathologically by preoperative fine needle biopsy or intraoperative excision biopsy; ⑥informed consent to endoscopic near-total thyroidectomy or conversion to open surgery and functional neck dissection.

排除标准：

①年龄≥61岁；②肿瘤侵及喉返神经引起声音嘶哑者；③术前检查或术中发现明显的淋巴结转移者；④术前检查发现肿瘤压迫或浸润性生长引起气管狭窄或变形；⑤曾有过甲状腺手术史；⑥患者不愿接受腔镜手术者；⑦妊娠期和哺乳期妇女。

Exclusion criteria：

①Age>=61 years;
②with hoarseness caused by tumor invasion of recurrent laryngeal nerve;
③with metastatic cervical lymph nodes;
④with tracheal stenosis or deformation caused by tumor compression or invasion;
⑤with history of thyroid surgery;
⑥rejection to endoscopic surgery; ⑦pregnant and lactating women.
