研究目的：

探讨局部消融技术在甲状腺良性结节治疗中的可行性、有效性及安全性；通过对比分析的方法评价射频、微波及激光消融在甲状腺结节治疗中的疗效、安全性；探讨联合应用不同消融技术治疗甲状腺结节的适应证及禁忌证，并对联合应用的安全性、及疗效做出临床评估  

Objectives of Study：

Evaluate the feasibility, effectiveness and security of local ablation in the treatment of benign thyroid nodules; analysis the efficacy and the safety of RF, microwave and laser ablation in the treatment of thyroid nodules by the comparative method; to make a clinical assessment for security, and efficacy of the combination of different ablation technique in the treatment of thyroid nodules,explore indications and contraindications for combination of different ablation technique.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

入选标准：（符合下列所有条件者可入选）
1)实性或以实性为主（>20%）结节
2)结节最大直径＞2cm
3)结节未行其他治疗（如：放射性碘治疗、TSH抑制治疗、酒精消融治疗等)
4)经2次针吸细胞学检查或粗针穿刺活检均符合良性结节
5）自身条件不能手术或拒绝手术
6）结节存在以下情况之一者：
①结节明显增长（1年内体积增大50%以上，或至少有2 条径线增加超过20%[并且超过2 mm])
②患者存在与结节明显相关的自觉症状（如：异物感、颈部不适或疼痛）
③结节明显外凸影响美观并要求治疗
④患者思想顾虑过重影响正常生活而拒绝临床观察
⑤自主功能性结节引起甲亢症状

Inclusion criteria

Inclusion criteria: (meet all of the following conditions can be selected)
1. nodule or predominantly (>20%)solid nodule;
2. maximum diameter of the nodule > 2cm;
3. nodules did not undergo other treatments (such as: radioactive iodine therapy, TSH suppressive therapy, ethanol ablation);
4. the pathologic result of 2 times needle aspiration or core needle biopsy is benign;
5. inoperable due to the patient's bad body conditions or refused to surgery;
6. nodules according to one of the following items below:
(1) nodules increased significantly (volume increases more than 50% within 1 year,or at least two diameters of the nodule increase over 20% and it more than 2 mm long]);
(2) patients with nodules significantly related symptoms (such as: foreign body sensation, neck discomfort or pain);
(3) nodules obviously convex affect the appearance and require treatment;
(4) ideological concerns of patients too heavy to affect normal life and denial of clinical observation;
(5) autonomously functioning nodules caused symptoms of hyperthyroidism.

排除标准：

排除标准：（符合下列任意一条即排除）
①滤泡性肿物或穿刺活检提示恶性
②病灶对侧声带功能不正常
③严重凝血机制障碍
④严重心肺疾病
⑤拒绝参与本试验或拒绝签署知情同意书

Exclusion criteria：

Exclusion criteria: (excluding comply with any of the following items)
1. the follicular nodules or biopsy suggest the malignancy;
2. the founction of contralateral vocal cord is not abnormal;
3. severe coagulation disorders;
4. severe cardiac or pulmonary disease;
5. refused to participate in this research or refused to sign an informed consent.
