研究目的：

初步探究XB计算系统所运算出的放射剂量对鼻咽癌患者垂体功能减退的影响。  

Objectives of Study：

This preliminary was to investigate the effect of radiation dose calculated by AcurosXB calculation system on hypophysis in the patients with nasopharyngeal carcinoma.

药物成份或治疗方案详述：

以2020年5月~2022年4月我科收治的经组织病理学确诊为鼻咽癌的初治患者为观察对象，根据入排标准筛选病人，签署知情同意书，完善基线检查包括垂体功能指标检查，根据诊疗指南制定治疗计划，包括单纯放疗、同步放化疗或同步放疗联合靶向治疗等，进行放疗定位CT扫描，勾画靶区，靶区勾画结束后患者按计划进行放疗，同时由物理师将靶区数据传输至香港理工大学胡永祥、梁允信教授团队，利用Acuros XB 高级剂量计算算法来精准计算鼻咽癌放疗时脑垂体的剂量分布，计算完毕后再将数据传回本科室。总放疗剂量进行至40GY时复查鼻咽部CT，评估是否需要修改靶区。需要修改靶区者，放疗医师按规范重新勾画靶区后进行放疗直至结束，同时将数据再次传输到香港利用Acuros XB 高级剂量计算算法再次计算垂体受量；若无需修改靶区，则继续按计划放疗直至结束。放疗结束时、3月、6月、9月、12月分别进行5次垂体功能检查。 

Description for medicine or protocol of treatment in detail：

From May 2020 to April 2022, we enrolled previously untreated patients with nasopharyngeal carcinoma who were confirmed by histopatholog,patients were selected according to the admission criteria according to the admission criteria.informed Consent signedbaseline examination included pituitary function indicators,treatment plan were determined by the NCCN guideline, including radiotherapy alone, concurrent chemoradiotherapy or concurrent radiotherapy combined with targeted therapy.patients received radiotherapy following by CT scan localization and target delineation;At the same time, the target area data will be transmitted to professor Wu Yongxiang and Professor Liang Yunxin's team of The Hong Kong Polytechnic University. The advanced dose calculation algorithm of Acuros XB will be used to accurately calculate the dose distribution of pituitary gland during radiotherapy of nasopharyngeal carcinoma. After the calculation, the data will be sent back to the undergraduate room. When the total radiotherapy dose was 40GY, the nasopharyngeal CT was reexamined to evaluate whether the target area should be modified. If the target area needs to be modified, the radiotherapy physician shall resketch the target area according to the standard and perform radiotherapy until the end. At the same time, the data will be transferred to Hong Kong again. If there is no need to modify the target area, radiotherapy will continue as planned until the end. Pituitary function was examined 5 times at the end of radiotherapy, march, June, September and December. 

纳入标准：

1)年龄≥18岁，≤75岁；
2)经组织病理学确诊为鼻咽癌；
3)为初治患者；
4)PS评分≤2分；
5)预计生存期≥6个月；
6)自愿签署知情同意书；
7)依从性好，满足随访条件。

Inclusion criteria

1) Aged >= 18 years,<= 80 years;
2) Nasopharyngeal carcinoma was confirmed by histopathology;
3) Untreated previously;
4) PS score <= 2 points;
5) Lifetime was expected >= 6 months;
6) Informed consent was signed;
7) Good compliance, meet the follow-up conditions.

排除标准：

1)既往有头颈部放疗病史；
2)有下丘脑、垂体相关的疾病；
3)有原发性内分泌疾病；
4)有精神疾病者；
5)激素替代治疗者；
6)妊娠和哺乳期女性；
7)ALB＜30g/L；
8)存在WBC＜4.0×10^9/L、血红蛋白＜80g/L、PLT＜50×10^9/L的患者；
9)反复高热尚未控制；
10)3 级、4 级血清肌酐值异常者；
11)3 级、4级谷丙转氨酶、谷草值异常者。

Exclusion criteria：

1) Previous history of head and neck radiotherapy;
2) Hypothalamus, pituitary gland related diseases;
3) Primary endocrine diseases;
4) People with mental illness;
5) Recent hormone replacement therapy;
6) Recent Pregnant and lactating female;
7) ALB < 30g/L;
8) WBC < 4.0 x 10^9/L, hematocrystallin < 80g/L, PLT < 50 x 10^9/L;
9）Uncontroled fever;
10）The abnormal serum creatinine level in grade 3 and 4;
11）The abnormal serum of ALT and /or AST in grade 3 and 4.
