研究目的：

针对重大的卫生公共事件、疫情防控、院前急救等特点条件下，定位识别颈动脉评估患者重要生命体征困难的问题展开研究，通过获取颈动脉超声图像序列，分割得到颈动脉血管和斑块的三维模型，获得一系列新的影像数据信息；再结合颈动脉及斑块多模态超声图像辅助诊断；开展前瞻性研究，构建和完善人工智能识别颈动脉系统，缩短评估急危重患者重要生命体征的时间。  

Objectives of Study：

Under the characteristics of major public health events, epidemic prevention and control, pre-hospital first aid, etc., it is difficult to locate and identify the carotid artery to assess the important vital signs of patients. The carotid artery blood vessel and plaque are segmented by acquiring the carotid artery ultrasound image sequence To obtain a series of new image data information; combined with multi-modal ultrasound images of carotid artery and plaque to assist diagnosis; carry out prospective research, construct and improve artificial intelligence to identify the carotid artery system, and shorten the critical life of critically ill patients The time of the signs.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

LAA组：
①首次发生的脑梗死，发病＜7 d；
②急性缺血性脑卒中TOAST分型为LAA；
③颈动脉软斑厚度＞2mm。
对照组：
①无心脑血管事件发作史的受试者（无临床症状组）；
②颈动脉软斑厚度＞2mm。

Inclusion criteria

LAA group:
1. The first cerebral infarction occurred and the onset was less than 7 days;
2, The TOAST classification of acute ischemic stroke was LAA;
3. The thickness of carotid soft plaque was more than 2mm.
Control group:
1. Subjects with no history of cardiovascular and cerebrovascular events (no clinical symptoms group);
2. Carotid artery soft plaque thickness> 2mm.

排除标准：

①使用超声造影剂的禁忌症，如不稳定型心绞痛、急性心力衰竭、急性心内膜炎；
②短暂性脑缺血发作；
③已知超声造影剂过敏者；
④近期活动性出血史，或伴恶性肿瘤、血液病、严重肝、肺、肾疾病。

Exclusion criteria：

1. Contraindications to the use of ultrasound contrast agents, such as unstable angina, acute heart failure, and acute endocarditis;
2. Transient ischemic attack;
3. Persons with known ultrasound contrast agent allergy;
4. Recent active bleeding History, or accompanied by malignant tumors, blood diseases, severe liver, lung, and kidney diseases.

脑梗死诊断参照全国第四届脑血管学术会议通过的诊断标准，并均经头颅MRI或CT证实。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The diagnosis of cerebral infarction was based on the diagnostic criteria adopted by the Fourth National Cerebrovascular Academic Conference, and was confirmed by MRI or CT.

选择2020年10月—2021年9月于上海市第一人民医院住院的急性动脉粥样硬化性脑梗死（large artery atherosclerosis, LAA）患者200例（LAA组）。

Select 200 patients with acute atherosclerotic cerebral infarction (large artery atherosclerosis, LAA) who were hospitalized in Shanghai First People's Hospital from October 2020 to September 2021 (LAA group).

容易混淆的疾病人群（即与目标疾病不易区分的一种或多种不同疾病，应避免采用正常人群对照的病例-对照设计）：

选取同期住院患者年龄、性别匹配的志愿者对照组200 例，受试者尿常规、肾功能正常，无心脑血管事件，无肝病及结缔组织病活动期。
