本项目将在明确AD危险因素综合干预是否能保护认知功能和降低痴呆发病的基础上，建立和完善一套适合我国独特社会、经济和文化特点的痴呆一级预防方案，并探讨针对脑组织结构性和功能性改变以及遗传易感性为基础的潜在病理学和生物遗传学机制。  

Objectives of Study：

This project aims to investigate whether the multimodal interventions on AD risk factors can protect the cognitive function and reduce the prevalence of dementia. We will further develop a set of dementia prevention programs which are suitable for the unique social, economic, and cultural characteristics of aging population in China. The underlying pathological and biogenetic mechanisms will be illustrated by structural and functional brain examination and genetic susceptibility.

药物成份或治疗方案详述：

符合入排标准的受试者以行政村为单位随机分为对照组、血管危险因素强化干预组和多模强化干预组。对照组仅接受国家基本公共卫生服务项目提供的医疗保健服务。血管危险因素强化干预组接受针对高血压、糖尿病以及血脂异常的强化随访、药物治疗和健康指导（每2个月监测一次血压和空腹血糖，每隔12个月监测一次血脂，每6个月一次群体健康教育），每次随访向受试者进行三种主要血管危险因素的基础健康教育。多模强化干预组是在血管危险因素强化干预方案基础上，定期组织健步走（每周组织集体步行活动5次，每次运动时长达20-30分钟），采用移动互联网云平台训练的形式进行认知训练（每周训练3次，每次20分钟，每次包含5个游戏，4分钟/游戏，每年集中训练6个月，共计72次），鼓励其多参加益智活动（向受试者提供剪纸和棋类游戏）。 

Description for medicine or protocol of treatment in detail：

Eligible participants for interventions in MIND-CHINA are divided randomly by village into one of the three groups: the control group, the vascular intervention group, and the multimodal intervention group. The control group receives regular health care services following the local governmental guidelines of primary public healthcare service program for the elderly. The vascular intervention group receives intensive monitoring, pharmacological treatment, and lifestyle and nutritional guidelines to improve treatment and control of hypertension, diabetes, and dyslipidemia (Blood pressure and fasting blood glucose are monitored every 2 months, blood lipids are monitored every 12 months, and group health education is held once every 6 months). Basic health education on the three main vascular risk factors is conducted to the subjects in each follow-up. In addition to medical control and healthy guidelines of three major vascular risk factors as planned in the vascular intervention group, the multimodal intervention group receives multidomain interventions that include group physical exercise (Group walking activities are organized five times a week with each exercise lasting for 20-30 minutes), cognitive training (Three times a week, 20 minutes each time, each time includes 5 games, 4 minutes/game, and the training is carried out for 6 months each year, a total of 72 times), and promotion of personalized leisure activities (Paper cutting and chess games are provided to the participants). 

纳入标准：

1、至基线调查时年龄在60-79岁；
2、在山东省阳谷县闫楼镇已长期居住≥1年。

Inclusion criteria

1. Aged 60-79 years at the time of baseline investigation;
2. People has been living in Yanlou town, Yanggu county of Shandong province for more than one year.

排除标准：

1、痴呆和失能者；
2、严重抑郁和精神分裂；
3、恶性疾病（如肿瘤）；
4、严重听力、视力、语言障碍无法完成认知量表者；
5、患有的疾病限制其参加干预活动者，尤其是体力锻炼活动（如、肝功异常、肾功异常、帕金森病、偏瘫、症状性心脑血管疾病以及经专业医师评定无法参加干预活动者）；
6、拒绝签署知情同意书者。

Exclusion criteria：

1. Patients with dementia or disability;
2. People with major depression or schizophrenia;
3. Severe disease such as malignant tumour;
4. Unable to complete the cognitive examination due to severe hearing, vision and/or language impairment;
5. Diseases limiting engagement in the intervention programs, especially physical activity/exercise (e.g.,hepatic dysfunction, renal dysfunction/failure, Parkinsons disease, paralysis, symptomatic cardiovascular and cerebrovascular diseases, and those who are unable to participate in intervention activities assessed by professional physicians);
5. Refuse to sign the inform consent form.

Statistical professionals will use SAS 9.4 software to generate random number seeds and randomly divided all the viiliages in Yanlou Town (52 villiages) into 3 groups using a cluster randomization method.

Blinding：

All subjects and researchers involved in the intervention or clinical evaluation did not know the grouping information. Blinding method will be throughout the intervention trial implementation process (including generation of randomization numbers, the randomized distribution, intervention effect assessment, experiment process monitoring, data management, statistical analysis). Until the data cleansing to quality acceptable level, it will be discovered to the appropriate personnel.

MIND-CHINA项目将采用纸质版的流行病学调查问卷，包含身体检查、认知评估和实验室检查，采用epidata软件将获得的信息录入电子信息系统里。
