研究目的:

有研究发现颈动脉校正血流时间和峰值流速呼吸变异度可作为成年人全麻诱导后低血压的预测指标。目前没有研究评估颈动脉校正血流时间和峰值流速呼吸变异度在预测老年高血压患者诱导后低血压方面的预测价值。本试验拟研究颈动脉校正血流时间和峰值流速呼吸变异度对老年高血压患者全麻诱导后低血压的预测价值。  

Objectives of Study:

Studies have found that carotid artery corrected blood flow time and peak flow rate respiratory variability can be used as predictors of hypotension after induction of general anesthesia in adults. There are currently no studies evaluating the predictive value of carotid artery corrected blood flow time and peak flow rate respiratory variability in predicting hypotension in elderly hypertensive patients after induction. This experiment intends to study the predictive value of carotid artery corrected blood flow time and peak flow rate respiratory variability in elderly hypertensive patients with hypotension after general anesthesia induction.

Description for medicine or protocol of treatment in detail:

 

纳入标准:

目前或既往被诊断为原发性高血压,全麻下行择期手术的患者;65-75岁之间,ASA Ⅱ-Ⅲ 级;体重指数(Body Mass Index,BMI):18-30kg/m2;颈动脉超声检测血管无斑块、狭窄和畸形等异常者;

Inclusion criteria

Patients currently or previously diagnosed with essential hypertension and undergoing elective surgery under general anesthesia; 65-75 years old, ASA Ⅱ-Ⅲ; Body Mass Index (BMI): 18-30kg/m2; neck Arterial ultrasound detection of blood vessels without abnormalities such as plaque, stenosis and malformation;

排除标准:

正在服用ACEI/ARB类降压药者;合并其它严重的外周血管疾病者;严重内分泌疾病;严重肝肾疾病;严重心肺疾病;基础平均动脉压(MAP)<70mmHg;

Exclusion criteria:

Those who are taking ACEI/ARB antihypertensive drugs; those with other severe peripheral vascular diseases; severe endocrine diseases; severe liver and kidney diseases; severe heart and lung diseases; basic mean arterial pressure (MAP) <70mmHg;

收缩压下降超过基础值的30%或平均动脉压下降超过基础值的20%,或收缩压小于90mmHg,或平均动脉压小于65mmHg。

Gold Standard or Reference Standard (The clinical reference standards required to establish the presence or absence of the target condition in the tested population in present study):

The systolic blood pressure drops more than 30% of the base value or the mean arterial pressure drops more than 20% of the base value, or the systolic blood pressure is less than 90mmHg, or the average arterial pressure is less than 65mmHg.
