研究目的：

通过评估口内径路舌根大部切除手术（简称舌根大部切除术）治疗阻塞性睡眠呼吸暂停低通气综合征（OSAHS）的手术疗效，探索舌根大部切除术患者的围手术期处理方案，手术范围、深度及其与疗效的关系，最终建立舌大部切除术治疗OSAHS的规范流程。  

Objectives of Study：

Through the evaluation of the surgical effect of the treatment of obstructive sleep apnea hypopnea syndrome (OSAHS) by the subtotal resection of the root of the tongue through the internal diameter of the mouth (abbreviated as the subtotal resection of the root of the tongue), to explore the perioperative treatment plan of the patients with the subtotal resection of the root of the tongue, the scope, depth of the operation and its relationship with the efficacy, and finally to establish the standardized process of the treatment of OSAHS by the subtotal resection of the tongue.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）18-60岁，性别不限；
（2）PSG监测诊断为中、重度OSAHS（AHI>15次/小时）的患者；
（3）呼吸机及口腔矫治器治疗失败、不耐受或拒绝者；
（4）电子喉镜检查、口咽CT扫描测量评估阻塞平面符合舌咽平面阻塞；
（5）Friedman分级II型以上，TCM评分17分以上；
（6）能接受手术可能的并发症、后遗症。

Inclusion criteria

(1) 18-60 years old, gender unlimited;
(2) Patients with moderate or severe OSAHS (AHI > 15 times / hour) were diagnosed by PSG monitoring;
(3) Failure, intolerance or rejection of ventilator and oral appliance treatment;
(4) Electronic laryngoscopy and oropharyngeal CT scan were used to evaluate whether the occlusive plane was consistent with that of glossopharyngeal plane;
(5) Friedman grade II or above, TCM score 17 or above;
(6) It can accept the possible complications, sequelae.

排除标准：

（1）全身状况差、严重心脑血管疾病无法耐受全麻手术者。
（2）严重影响气道的头面部骨性畸形或异常。
（3）妊娠期、甲状腺功能低下、肢端肥大症及其他内分泌失调导致的睡眠障碍者。
（4）严重肥胖，BMI>40kg/m2者。
（5）滥用药物或者酒精成瘾或有难以控制的精神疾病者。
（6）对手术疗效预期不符合实际者。
（7）智力障碍或智力不成熟，行为不能自控者。

Exclusion criteria：

(1) The patients with poor general condition and severe cardiovascular and cerebrovascular diseases can not tolerate general anesthesia.
(2) The head and face bone deformity or abnormality that seriously affects the airway.
(3) Sleep disorders caused by pregnancy, hypothyroidism, acromegaly and other endocrine disorders.
(4) Severe obesity, BMI > 40 kg / m2.
(5) People who are addicted to drugs or alcohol or have mental disorders that are difficult to control.
(6) Those who do not meet the expectation of the surgical effect.
(7) The person who is mentally retarded or immature and whose behavior cannot be controlled.
