研究目的：

评估肺静脉前庭放射状线性消融术式治疗阵发性房颤的远期预后和安全性。肺静脉前庭放射状线性消融术目的是疏导分流源于肺静脉的异位电流。而环肺静脉消融则只是围堵隔离肺静脉异位电流。这两种术式采用了完全不同的哲学理念。  

Objectives of Study：

To evaluate the efficacy and safety of pulmonary antrum radial-linear ablation on paroxysmal atrial fibrillation. Pulmonary antrum radial-linear ablation aims to shunt the pulmonary ectopic impulse. While circumferential pulmonary vein ablation is to isolate the pulmonary ectopic impulse. The concept of philosophy related to both the strategies is much different.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.性别不限，年龄18至78岁；
2.能够且愿意提供书面知情同意书；
3.有心慌症状且抗心律失常药物治疗无效的阵发性房颤；
4.左房前后径不大于45mm。

Inclusion criteria

1.Male and female aged 18 to 78 years old.
2.Able and willing to provide signed informed consent.
3.Patients with drug refractory recurrent symptomatic paroxysmal atrial fibrillaton.
4.Left atrial diameter no more than 45mm.

排除标准：

1.继发于电解质紊乱、甲状腺疾病、或可逆性或非心源性原因的房颤；
2.6个月内接受过冠脉搭桥手术者；
3.影像学（如食道超声）提示左房血栓；
4.1年内曾发生血栓栓塞事件者；
5.左房粘液瘤；
6. 孕妇；
7. 急性疾病或急性全身感染或败血症；
8.不稳定心绞痛；
9.不能控制的心衰；
10.2月内心肌梗死者；
11.凝血功能异常者；
12.有抗凝禁忌证者；
13.血管内血栓、肿瘤或其他异常以致不能接受导管插入或操作者。

Exclusion criteria：

1.Atrial fibrillation secondary to electrolyte imbalance, thyroid disease, or reversible or non-cardiac cause.
2.CABG procedure within the last six (6) months.
3.Documented left atrial thrombus on imaging (e.g. TEE).
4.History of a documented thromboembolic event within the past one (1) year.
5.Diagnosed atrial myxoma.
6.Women who are pregnant.
7.Acute illness or acute systemic infection or sepsis.
8.Unstable angina.
9.Uncontrolled heart failure.
10.Myocardial infarction within the previous two (2) months.
11.History of blood clotting or bleeding abnormalities.
12.Contraindication to anticoagulation (i.e. heparin or warfarin).
13.Presence of intramural thrombus, tumor or other abnormality that precludes catheter introduction or manipulation.

肺静脉前庭放射状线性消融组: 在两侧肺静脉前庭做6-8条放射状线性消融，从肺静脉口开始，到前庭-左房交界部（(此处只能记录到单纯左房电位)）为止，以完成预设消融径线且房颤不被诱发为消融终点。

Intervention：

Pulmonary antrum radial-linear ablation group: six to eight radial-linear lesions were created with RF ablation from pulmonary vein orifice to pulmonary vein –left atrium junction( the site where only left atrial potential was present). The procedural end point was the completion of all the designed radial-linear lines of lesions and atrial fibrillation became noninducible.
