研究目的：

1.提高中青年高血压患者自我健康管理能力；从而提高高血压的控制率，进而提升其生活质量。 2.规范社区健康管理服务，推广“互联网+”高血压管理，实现双平台管理，优化慢病管理服务流程。 3.积极探索和大力推广上下联动的医疗联合体制机制，让患者就近、充分的享受优质医疗服务资源，实现基于医院分级的同质化健康管理服务。  

Objectives of Study：

1. Improve the self-health management ability of young and middle-aged patients with hypertension;Thus, the control rate of hypertension is improved and the quality of life is improved. 2. Standardize community health management services, promote "Internet +" hypertension management, realize dual-platform management, and optimize chronic disease management service process. 3. Actively explore and vigorously promote the linkage of medical joint system and mechanism, so that patients can enjoy nearby and fully high-quality medical service resources, and realize the homogenized health management services based on hospital classification.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.根据世界卫生组织（WHO）年龄分类标准，年龄在 18～59 岁；
2.已确诊的原发性中青年高血压患者；
3.能较为熟练地应用智能手机，且使用微信；
4.常住居民（在社区居住不少于 6 个月）；
5.自愿参加本研究并签署知情同意书，同意接受本课题组各 种干预方法、服从课题组安排者。

Inclusion criteria

1. According to the World Health Organization (WHO) age classification standard, the age is between 18 and 59 years old;
2. Diagnosed patients with primary hypertension;
3. Able to skillfully use smart phones and use WeChat;
4. Permanent residents (living in the community for at least 6 months);
5. Voluntarily participate in this study, sign informed consent, agree to accept various intervention methods of our research group, and obey the arrangement of our research group.

排除标准：

1.继发性高血压及有其他器官并发症；
2.合并有其他严重躯体性疾病；
3.合并有其他神经系统疾病以致精神状态异常；
4.有精神性疾病家族史,有药物或酒精依赖史。

Exclusion criteria：

1. Secondary hypertension and other organ complications;
2. Complicated with other serious somatic diseases;
3. With other neurological diseases resulting in abnormal mental state;
4. Family history of mental illness, drug or alcohol dependence.
