研究目的：

本课题以提高临床疗效为目的，遵循中医药学科特点，通过规范化临床研究，建立优化的HBeAg阳性慢性乙型肝炎中西医结合治疗方案，改善该部分病人的结局指标，降低病毒性肝炎的发病率及病死率，提高对乙型肝炎的疾病控制能力，服务于专项的总体目标。  

Objectives of Study：

Aim to use the optimized scheme of integrated Chinese medicine and conventional medicine to treat the chronic hepatitis B patients with positive HBeAg. It is hypothesized that the optimized scheme will improve partial outcomes, reduce the mortality induced by hepatitis B and promote the capacity of the hepatitis control.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

(1)符合HBeAg阳性慢性乙型肝炎的诊断标准；
(2)年龄在18-65岁之间；
(3)血清HBV-DNA阳性；
(4)肝功能及HBeAg或肝脏病理满足以下条件之一：
①2×ULN≤ALT≤3×ULN，且HBeAg 50-100 S/CO（约等于11.63-24.26 PEIU/ml）；
②2×ULN≤ALT≤3×ULN，且HBeAg700-1250 S/CO（约等于190.82-352.81 PEIU/ml）(该部分病例数不少于总病例数的60%)；
③3×ULN≤ALT≤5×ULN，且HBeAg≤50 S/CO（约等于11.63 PEIU/ml）；
④5×ULN≤ALT≤7×ULN，且HBeAg 250-400 S/CO（约等于64.07-105.43 PEIU/ml）；
⑤ALT<2×ULN，但肝组织学显示Knodell HAI≥4，或炎症坏死≥G2，或纤维化≥S2；
(5)TBIL≤3×ULN；
(6)中医辨证为“肝郁脾虚 肝胆湿热”复合证型。
HBeAg阳性慢性乙型肝炎的诊断标准：参照《慢性乙型肝炎防治指南》(中华医学会肝病学分会、中华医学会感染病学分会联合制订，2010年12月)。
HBeAg阳性慢性乙型肝炎：血清HBsAg、HBeAg阳性，抗-HBe阴性，HBV DNA阳性，ALT持续或反复升高，或肝组织学检查有肝炎病变。
慢性乙型肝炎中医辨证诊断标准：参照世界中医药学会肝病委员会.慢性乙型肝炎（ALT≥2×ULN）中医证候诊断标准（草稿） [C].世界中联第四届肝病国际学术大会暨海峡两岸中医肝病研讨会论文集,厦门,2011.291-292. 拟定。

Inclusion criteria

(1)Match the diagnositc standard of HBeAg positive hepatitis B;(2)Aged 18-65 years;(3)Positive HBV-DNA of serum test;(4)Liver function and HBeAg or live pathology match of the following situations:①both 2×ULN≤ALT≤3×ULN and HBeAg 50-100 S/CO (equal to 11.63-24.26 PEIU/ml);②both 2×ULN≤ALT≤3×ULN and HBeAg700-1250 S/CO (equal to 190.82-352.81 PEIU/ml) and the cases should not less than 60% of total number of participants;③both 3×ULN≤ALT≤5×ULN and HBeAg≤50 S/CO (equal to 11.63 PEIU/ml);④both 5×ULN≤ALT≤7×ULN and HBeAg 250-400 S/CO (equal to 64.07-105.43 PEIU/ml);⑤ALT<2×ULN and Knodell HAI≥4 or atory necrosis≥G2, or rate of liver fibrosis ≥S2；(5)TBIL≤3×ULN;(6)With complexed TCM "zheng" of “Ganyu Pixu (liver melancholy and spleen deficincy)", "Gandan Shire (liver and gallbladder hot and humid".The diagnositc standard of HBeAg positive hepatitis B: based on the "Guidance of Prevention and Treatment of Chronic Hepatitis B" by Chinese Medical Sociate Liver Diseases branch, Chinese Medical Sociate Infectious Diseases Branch, 2010 Dec.);Definition of HBeAg positive hepatitis B: serum HBsAg and HBeAg positive, anti-HBe negative, HBV DNA positive, ALT continue or repeadly increasing, or abnormal in the histological examination of the liver;Diagnositc standard of TCM "bian zheng" for chronic hepatitis B: The World TCM Association Liver Committee: Chronic Hepatitis B (ALT≥2×ULN), TCM signs standard [C]. The Conference Book of the Fourth International Liver Diseases Conference of World TCM Sociate and Mainland-Taiwan TCM Liver Diseases Conference, Xiamen, 2011.291-292.

排除标准：

(1)妊娠或准备妊娠妇女，哺乳期妇女，男性准备授孕者；
(2)血清学检测证实由肝炎病毒重叠感染、非病毒感染(如酒精性肝病)引起的慢性肝炎，或隐源型肝炎者；
(3)合并肝硬化（肝纤维化S4以上）、肝癌、肝性脑病、水电解质及酸碱平衡紊乱、消化道出血、感染的慢性乙肝严重并发症者；
(4)慢性乙肝重度和慢性重型肝炎；
(5)原发性和/或继发性心脑血管、肺、肾、内分泌、神经和血液系统疾病者；
(6)一个月内参加过或正在参加其他临床试验者；
(7)精神异常不能表达自身感受者；
(8)乙肝病毒变异者；
(9)对恩替卡韦存在耐药变异者。

Exclusion criteria：

(1)Pregnancy or preparing to pregnance women, lactating women, man preparing to insemination;
(2)Confirmed multiple infection of hepatitis virus by serum test, none virus caused chronic liver diseases such as Alcoholic liver disease, or cryptogenic hepatitis;
(3)combined with cirrhosis (liver fibrosis above S4), liver cancer, hepatic encephalopathy, Water electrolyte and acid-base balance disturbance, gastrointestinal bleeding, severe compications with chronic hepatitis;
(4)Severe chronic hepatitis B and chronic severe heparitics;
(5)Orginal and/or Secondary cardiovascular and cerebrovascular, lung, kindney, endocrine system, nerve system and blood system diseases;
(6)To be participated other clinical studies in past one month;
(7)Mental disorder unable to express the own feeling;
(8)Mutation of hepatitis B virus;
(9)To be with drug resistance variation against Entecavir.
