研究目的：

消化性溃疡是目前常见病，发病率呈上升趋势，随着溃疡治疗药物的研发，大部分消化性溃疡经过内科治疗后均可获得治愈，但仍有小部分消化性溃疡仍需外科手术干预，虽然有外科腹腔镜的应用，如能避免外科手术可减少了患者痛苦，降低住院费用，减少一系列术中及术后并发症的出现。针对部分内科治疗效果欠佳、消化性溃疡伴活动性出血、累及血管、溃疡面有陈旧性血痂附着、巨大溃疡、胃十二指肠复合溃疡、凹陷性溃疡深达浆膜、胃体上部溃疡等进行内镜下荷包缝合术，与单纯内科药物治疗疗效进行对比，评估其治疗效果。  

Objectives of Study：

Peptic ulcer is a common disease, the incidence is rising. With the development of ulcer drugs, most peptic ulcers can be cured after medical treatment, but there are still a small part of peptic ulcer need surgical intervention. Although the applications of laparoscopic surgical can be chosed to avoid surgery, to reduce the suffering of patients and reduce hospital costs, and the occurrence of a series of intraoperative and postoperative complications. For some cases that medical treatment is ineffective, this study aims to compare the effects of endoscopic suture purse with internal medicine therapy in the treatment of peptic ulcer with active bleeding, involving the blood vessels, ulcers had old blood scab attached, great ulcer, gastroduodenal ulcer, pitting ulcer deep serous, the upper part of the stomach ulcer.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.既往有溃疡病史，药物保守治疗效果差；2.消化性溃疡伴活动性出血、累及血管、溃疡面有陈旧性血痂附着；3.巨大溃疡（直径>2.0cm)；4.胃十二指肠复合溃疡；5.凹陷性溃疡深达浆膜层；6.胃体上部溃疡

Inclusion criteria

1. The previous history of ulcers, conservative medical treatment is poor;
2. Peptic ulcer with active bleeding, involving the blood vessels, ulcers had old blood scab adhesion;
3 huge ulcers (diameter> 2.0cm);
4. gastroduodenal ulcer;
5. depressed ulcer deep serosal;
6. upper gastric ulcers.

排除标准：

1.年龄大于75岁；2.严重心肺功能障碍；3.凝血功能障碍；4.排除肝硬化、门脉高压性胃病；5.恶性溃疡

Exclusion criteria：

1. Age greater than 75 years old;
2. severe cardiopulmonary dysfunction;
3. coagulation disorders;
4. exclude cirrhosis, portal hypertension gastropathy;
5. malignant ulcer.
