研究目的：

探讨手动、机械腹部按摩对重症脑卒中行EN者胃肠消化功能和吸收功能的影响；探讨为重症脑卒中行EN患者提供手动、机械腹部按摩所需的干预人力成本和经济成本；旨在改善重症脑卒中行EN患者的胃肠道功能，为临床转化以及科室决策提供相关建议。  

Objectives of Study：

To explore the effect of manual and mechanical abdominal massage on gastrointestinal digestion and absorption function in EN patients with severe stroke; to explore the human cost and economic cost of intervention to provide manual and mechanical abdominal massage for EN patients with severe stroke; to improve the gastrointestinal function of patients with severe stroke and to provide relevant suggestions for clinical transformation and department decision-making.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

年龄18-80岁；经医师根据2010年中华医学会神经病学分会脑血管病组的诊断标准确诊为脑卒中且收入ICU者；不能经口进食、洼田饮水试验3级者；入院后 48h 内可经鼻胃管进行肠内营养者；患者或家属愿意参与本次研究，并签署知情同意书者；

Inclusion criteria

Age is 18-80 years; According to the diagnostic criteria of cerebral vascular disease group of neurology branch of chinese medical association in 2010, the doctor diagnosed stroke with icu; Not eat through the mouth or lowland drinking water test for grade 3; Enteral nutrition through nasogastric tube within 48 h of admission; Patients or family members willing to participate in the study and sign informed consent;

排除标准：

入组前有恶心、呕吐症状者；6周内接受过腹部手术和放射治疗者；合并肿瘤、脊髓损伤者；肠内营养和肠外营养联合使用者；腹部按摩禁忌症者（怀孕、消化道出血、肠梗阻）；病情加重、停止肠内营养或提前转出ICU者；

Exclusion criteria：

Symptoms of nausea and vomiting before admission; Abdominal surgery and radiotherapy within 6 weeks; Complicated with tumor or spinal cord injury; Combined enteral and parenteral nutrition users; Contraindications for abdominal massage (pregnancy, gastrointestinal bleeding, intestinal obstruction);Aggravation of disease or cessation of enteral nutrition or early transfer of icu;

研究期间发生的胃潴留、腹胀、便秘、腹泻及返流（呕吐）定义为胃肠道并发症。本研究中一次性抽出GRV＞200ml即为胃潴留。腹胀是采用腹围值及腹部的深浅触诊法来评估腹胀，若腹围较鼻饲前增加，且采用触诊的方法对腹部进行评估腹部触诊较硬、移动度降低且紧张度增高即可判断为腹胀。患者行肠内营养后3天未解大便即被定义为便秘。腹泻是指一天的排泄次数≥3次，且粪便为水样便，水分大于85%。返流是指胃内容物经口鼻流出，因重症脑卒中患者的呕吐和返流无法明确区分，通常将呕吐和返流一起进行评估。
