基于GMs和MRI预测高危儿神经发育结局。  

Objectives of Study：

The purpose of this study was to predict neurodevelopmental outcomes in high-risk infants based on GMs and MRI.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）至少有一次不安运动阶段的GMs评估；
（2）矫正足月内头颅MRI检查；
（3）随访至１周岁后，有明确的运动发育结局诊断。
具有以下高危因素的新生儿：
1) 胎龄＜37周；
2)出生体重＜2500ｇ；
3) 出生Apgar评分0～5分；
4）双胎、多胎；
5）母亲妊娠期曾患糖尿病、妊高症、甲亢、肝肾等疾病；
6）有医学指征的剖宫产；
7）新生儿期有颅内出血、中枢神经系统感染等；
8）重症黄疸，新生儿血清总胆红素≥20mg/dＬ（核黄疸）或ABO溶血症患儿；
9）生后４月龄内患过严重疾病（化脑、颅内出血、缺血缺氧性脑病等）；
10）家长发现４月龄内婴儿运动发育可疑者。

Inclusion criteria

(1) High-risk neonates who had visited our department were evaluated by GMs at least one assessment of the stage of restless movement;
(2) All the subjects had completed structural skull MRI within a month of correction;
(3) Follow-up up to 1 year old (correction) had clear motor development outcomes according to clinical neurological manifestations and developmental assessments.
Infants with the following risk factors:
1. Gestational age < 37 weeks;
2. Birth weight <2500 g;
3. Apgar score at birth ranged from 0 to 5;
4. Twins, multiple birth;
5. During pregnancy, the mother had suffered from diabetes mellitus, hyperthyroidism, liver and kidney diseases;
6. Cesarean section with medical index;
7. Neonatal period had intracranial hemorrhage, central nervous system infection;
8. Severe jaundice, neonatal total bilirubin >=20mg/dL (nuclear jaundice) or ABO hemolysis;
9. Had a serious illness within 4 months of birth;
10. Infants with suspicious motor development within 4 months of age.

排除标准：

确诊遗传代谢性疾病者。

金标准或参考标准（即可准确诊断某疾病的单项方法或多项联合方法，在本研究中用于诊断是否有该病的临床参考标准）：

1.脑瘫：诊断标准按照2006年国际脑瘫定义，在随访对象12月龄以后在本院康复门诊经临床神经学检查确定为脑瘫，或经电话随访纳入对象12月龄以后在其他医院神经科或康复科确诊为脑瘫。 2.运动发育迟缓：纳入对象12月龄以后在本院经运动发育PDMS量表评估或0-6岁儿童发育评估量表评估GMQ和或FMQ小于80分，并且临床神经学检查排除脑瘫者。 3.运动发育正常：纳入对象12月龄以后在本院经运动发育PDMS量表评估或0-6岁儿童发育评估量表评估GMQ和FMQ均大于等于80分，并且临床神经学检查排除脑瘫者。
