研究目的：

理论方面：研究采用静息态功能磁共振评估认知行为治疗对抑郁症患者疗效、转归及其自杀风险变化的影响，从影像学角度寻找认知行为治疗对抑郁症的疗效预测因子。 临床方面：抑郁症患者是自杀的高危人群，认知行为治疗是预防自杀的重要心理治疗措施。但目前关于认知行为治疗对其产生疗效的神经生物学机制还不明确。本研究将探索认知行为治疗干预对神经网络环路的影响，为认知行为治疗干预的疗效预测及选择提供客观的影像学依据，因此具有较大的临床意义。  

Objectives of Study：

Theoretical: in this study, resting functional magnetic resonance imaging (fMRI) was used to evaluate the effects of CBT on the efficacy, outcome and suicide risk of patients with depression, and to search for the predictive factors of CBT on the efficacy of depression from the perspective of imaging. Clinical: depression patients are the high-risk group of suicide, cognitive behavioral therapy is an important psychological treatment to prevent suicide. However, the neurobiological mechanism of the effect of cognitive behavioral therapy is still unclear. This study will explore the influence of cognitive behavioral therapy intervention on the neural network loop, so as to provide objective imaging basis for the prediction and selection of the therapeutic effect of cognitive behavioral therapy intervention, so it has great clinical significance.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

（1）汉族，年龄18-50岁，右利手，文化程度为初中及以上；
（2）诊断为“抑郁发作”（诊断标准为美国精神障碍诊断和统计手册第4版（DSM-IV））
（3）汉密尔顿抑郁量表(24项)评分≥35分；
（4）患者入组前1个月内未接受过抗抑郁药物治疗，无电休克治疗史及经颅磁刺激治疗史。

Inclusion criteria

(1) Han nationality, aged 18-50 years, right-handed, junior high school or above;
(2) Diagnosed as a "depressive episode" (diagnostic criteria: American diagnostic and statistical manual of mental disorders version 4 (dsm-iv));
(3) Hamilton depression scale (24 items) score >= 35;
(4) The patients had not received antidepressant treatment, no history of electroconvulsive therapy and no history of transcranial magnetic stimulation therapy within 1 month before enrollment.

排除标准：

（1）伴有其他精神障碍，如人格障碍、焦虑障碍、精神发育迟滞、躁狂发作、精神分裂症等；
（2）汉密尔顿焦虑量表（Hamilton Anxiety Scale，HAMA）评分≥14分；
（3）伴脑器质性疾病；
（4）抑郁症状为其它器质性疾病或药物所导致；
（5）有物质依赖史；
（6）存在MRI扫描禁忌、妊娠期、哺乳期及准备妊娠的女性；
（7）合并严重躯体疾病。 注：根据哥伦比亚自杀评估分类法（Columbia Classification Algorithm for Suicide Assessment, C-CASA） ，本研究中自杀未遂定义为：“有意愿结束自己生命，已采取自我伤害行为但未成功，有明确的自杀企图证据或是可以从其行为及境遇推论得出”。
中止标准： （1）出现诊断修改； （2）不能完成家庭作业或缺席CBT超过2周； （3）被试者要求退出本研究。

Exclusion criteria：

(1) Accompanied by other mental disorders, such as personality disorder, anxiety disorder, mental retardation, manic episode, schizophrenia, etc.;
(2) Hamilton Anxiety Scale (HAMA) score 14;
(3) Accompanied by brain organic diseases;
(4) Depressive symptoms are caused by other organic diseases or drugs;
(5) A family history of substance dependence or mental illness;
(6) Women with contraindications in MRI scanning, pregnancy, lactation and pregnancy preparation;
(7) Complicated with severe physical diseases. Note: according to the Columbia Classification Algorithm for Suicide Assessment (c-casa), Suicide attempt in this study is defined as: "a person who has the intention to end his/her life, has taken self-harming behavior but has not succeeded, has clear evidence of Suicide attempt or can be inferred from his/her behavior and circumstances".
Termination criteria:
(1) Diagnostic modification occurs;
(2) Unable to complete homework or absent from CBT for more than 3 weeks;
(3) Subjects are required to withdraw from this study.

本研究通过随机数字表来进行随机化分组，将符合诊断标准的抑郁症患者分为合用认知行为治疗组（CBT组）和单纯抗抑郁药物治疗组（MG组），分组操作具体步骤如下： ① 编号：将符合入组标准并签署知情同意书的患者按就诊顺序进行编号； ② 获取随机数：按照被试者编号从随机数字表中得到每个被试者的随机数字； ③ 分组：根据被试者获得的随机数字的奇偶将其分为CBT组和MG组。
