研究目的：

1、基于分级诊疗制度及医联体协同服务，探讨云平台技术支撑的急性冠脉综合征区域共治心脏康复管理模式的可行性。 2、创新冠心病慢病联防联控管理模式，探讨新模式下医联体团队急性冠脉综合征心脏康复管理与服务的提供能力及人民群众的健康获得感。 3、基于疾病全程管理视角，探讨急性冠脉综合征心脏康复延续健康管理的有效策略，丰富急性冠脉综合征专科护理内涵，为改善急性冠脉综合征预后提供依据。  

Objectives of Study：

This study aimed to evaluate the possible positive effects of the risk stratification intelligent follow-up management system (RSIFMS) on the disease management and cardiac rehabilitation of discharged ACS patients.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

符合急性冠脉综合征符合2004版及2007版ACS诊断标准；年龄>20周岁；没有精神或心理疾病，可以正常沟通交流；镇江市市区常住人口，同意电话随访或家庭访视。

Inclusion criteria

(a) diagnosed with ACS for 2004 and 2007 versions of ACC/AHA, (b) aged >20 years, (c) had no mental problems, (d) were able to communicate, and (e) can be contacted by telephone and home visit.

排除标准：

无法控制的心律心失常或严重的多器官功能衰竭。

Exclusion criteria：

uncontrolled arrhythmia or severe multiple organ failure.
