研究目的：

胸脊柱段带状疱疹后遗神经痛（PHN）占总PHN发生率的50%，本课题选择老年胸背部PHN患者为受试对象，拟采用脉冲射频肋角处胸神经前支、脉冲射频胸神经后支、脉冲射频胸背根神经节、脉冲射频胸神经前支加后支四种治式镇痛.本研究论证①胸神经后支敏化也参与PHN的发痛机制；②同时脉冲射频胸神经前支与后支可能获得协同性的镇痛效果；③脉冲射频肋角处胸神经前支能否达到脉冲射频胸背根神经节的疗效；④同时脉冲射频胸神经前支与后支的疗效彻底、安全高，可能优于脉冲射频胸背根神经节。如能取得突破性发现，可为老年PHN患者提供更加有效的脉冲射频治疗新方法，这对消除严重PHN引起的身心障碍，提高老年患者的生存质量具有重要的临床意义。  

Objectives of Study：

The thoracic nerve (T1-12) is the most common region affected in PHN with an incidence of to about 50%. Senile thoracic nerve PHN patients were chosen to be subject in our study, four therapies of analgesia will be used in this study: 1. PRF on anterior ramus of thoracic nerve on angulus costae; 2. PRF on posterior ramus of thoracic nerve; 3. PRF on dorl root ganglion(DRG); 4. PRF on anterior posterior ramus of thoracic nerve. The purpose of this study is to demonstrate the following: 1. The sensitization of posterior ramus of thoracic nerve contributes to pain-inducing mechanism of PHN; 2. synergistic -analgesia may be achieved by simultaneously putting PRF on anterior posterior ramus of thoracic nerve; 3. whether the therapeutic effect of PRF on posterior ramus of thoracic nerve on angulus costae is me as DRG-targeting PRF; 4. Therapeutic effectiveness fety of PRF on anterior posterior ramus of thoracic nerve may be superior to conventional DRG-targeting PRF.The study will provide an effective method for application of PRF in the treatment of thoracic PHN if we could make a breakthrough in this field. It would be of great clinical significance to reduce psychosomatic disorder resulting severe PHN improve quality of life of senile patients.

纳入标准：

（1）带状疱疹位于T2-11躯体神经支配区
（2）患者年龄≥60岁
（3）急性带状疱疹病史≥1月
（4）VAS≥60mm

Inclusion criteria

1. Patients with thoracic PHN(T2-11);
2. Patients >=60 years old;
3. History of acute herpes zoster over one month;
4. VAS>=60mm.

排除标准：

（1）合并严重心、肝、肺、肾功能不全、
（2）凝血功能障碍患者
（3）智能障碍及配合欠佳患者

Exclusion criteria：

1. Severe heart, liver, lung kidney disease;
2. Patients with coagulation disorders;
3. Intellectually challenged and/or unable to follow instruction.
