研究目的：

近年来国内外广泛研究的一种动静脉内瘘穿刺后的止血方法，临床应用较多的压迫止血方法是使用压块（卷成硬芯的医用纱布块、消毒棉块或卫生纸卷），外加弹力压脉带环扎固定促进止血。止血过程中，为减少静脉回流受阻及保护内瘘血管，需要患者自行逐渐减压，每间隔10分钟左右放松一次压脉带，直至针眼处无渗血方可摘除压脉带。透析患者自行放松压脉带是用非内瘘侧手单手操作，手指配合固定压块的同时打开压脉带，在观察不出血的情况下再次环扎固定。由于动作比较复杂，加之血管硬化变性易滑动，单手操作极易发生按压位置偏离，造成皮下渗血、皮下血肿、穿刺点渗血和假性动脉瘤，造成内瘘闭塞，最终导致血管通路丧失功能。 在血液透析患者中使用带调节阀的止血气垫，客观评价方案的临床疗效，形成可在临床推广的、拔针后使用的规范的用品。  

Objectives of Study：

In recent years, a kind of hemostasis method after arteriovenous internal fistula puncture has been widely studied at home and abroad. The compression hemostasis method which is widely used in clinic is to use compression block (medical gauze block rolled into hard core, sterilized cotton block or toilet paper roll), plus elastic compression pulse belt for encircling fixation to promote hemostasis. In the process of hemostasis, in order to reduce the obstruction of venous return and protect the blood vessels of internal fistula, patients need to gradually reduce pressure by themselves, relax the pressure pulse band every 10 minutes or so, and remove the pressure pulse band until there is no blood seepage at the eye of the needle. In dialysis patients, the pressure band was released by one hand operation on the non internal fistula side, the pressure band was opened while the finger was used to fix the pressure block, and the pressure band was fixed again without bleeding. Because of the complex action and the easy sliding of the vascular sclerosis, the single hand operation is easy to deviate from the compression position, resulting in subcutaneous blood seepage, subcutaneous hematoma, blood seepage at the puncture point and pseudoaneurysm, resulting in the closure of the internal fistula, and eventually the loss of function of the vascular access. The hemostatic air cushion with regulating valve is used in hemodialysis patients to objectively evaluate the clinical effect of the scheme and form the standard articles that can be popularized in clinical practice and used after pulling out the needle.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1. 西医诊断为慢性肾衰，年龄18—80 岁；
2. 母语为汉语，自体动静脉内瘘；
3. 规律透析3个月以上,最近一个月未出现过止血异常（止血时间延长和或渗血、血肿及动脉瘤等并发症）；
4. 小学以上文化程度；
5. 神志清晰,无其他认知功能障碍；
6. 可配合 20 分钟的操作培训。

Inclusion criteria

1. Chronic renal failure patients aged 18 to 80 years old;
2. Native Chinese, arteriovenous fistula;
3. Regular dialysis for more than 3 months, no abnormal hemostasis (prolonged hemostasis time or complications such as bleeding, hematoma and aneurysm) occurred in the last month;
4. Education level above primary school;
5. Clear mind, no other cognitive dysfunction;
6. Can cooperate with 20 minutes of operation training.

排除标准：

1. 严重构音障碍、听觉障碍、认知障碍等原因导致患者不能配合量表评定；
2. 由于意识障碍和普通智力减退造成的语言障碍；
3. 因先天或幼年疾病引致学习困难，并造成的语言机能缺陷；
4. 因心肝肾等重要器官功能损伤，研究者认为不适合参加本研究者；
5. 广泛的动脉瘤，内瘘压力过大，长期口服抗凝药物和或实验室检查血小板计数异常、凝血功能障碍者，纳入研究后亦不能停止该疗法者。

Exclusion criteria：

1. Severe dysarthria, hearing impairment, cognitive impairment and other reasons lead to patients unable to cooperate with the scale evaluation;
2. Language disorder caused by consciousness disorder and general mental retardation;
3. Learning difficulties caused by congenital or juvenile diseases and resulting in language dysfunction;
4. Because of the damage of heart, liver, kidney and other important organs, the researchers think it is not suitable to participate in this study;
5. Patients with extensive aneurysms, excessive pressure of internal fistula, long-term oral anticoagulant drugs, or laboratory examination of platelet count abnormalities, coagulation dysfunction, who could not stop the treatment after being included in the study
