研究目的：

通过前瞻性、多中心、随机对照临床研究，比较动脉介入化疗和静脉化疗治疗单眼D、E期视网膜母细胞瘤的安全性和有效性。  

Objectives of Study：

A prospective, multicenter, randomized controlled clinical study, to compare the effectiveness and security of arterial interventional chemotherapy and venous chemotherapy in treating unilateral retinoblastoma of group D or E.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1) 新发单眼视网膜母细胞瘤,根据ICRB诊断标准，患眼属于D期或E期且不伴临床高危因素；
2) 骨髓功能正常：外周血嗜中性粒细胞绝对值 (ANC) ≥ 750/μL；血小板 > 75,000/μL； 血红蛋白≥ 8.0 gm/dL；
3) 肝功能正常：总胆红素≤1.5 x UNL (正常年龄的上线 )；天门冬氨酸氨基转移酶(AST) 和丙氨酸氨基转移酶(ALT)＜ 5 x UNL；白蛋白 ≥ 2 g/dL；
4) 肾功能正常：肌酐清除率（Creatinine clearance rate，Ccr）≥ 70ml/min/1.73 m2 或者根据年龄和性别血清肌酐（serum creatinine，Scr）的参考浓度；
5) 凝血功能正常: 国际标准化比值(international normalized ratio,INR)INR＜1.4；（Activated partial thrmoboplatin time）APTT＜ 34s；
6）自愿参加本临床实验，并签署知情同意书；有随访意愿，且能按照规定的时间进行随访。

Inclusion criteria

1) New onset unilateral retinoblastoma of group D or E without high-risk clinical factors
2) Normal bone marrow function: ANC >=750/uL; Plt >75,000/uL; Hb>=8.0 gm/dL;
3) Normal liver function: total bilirubin <=1.5*UNL (upper normal limit); AST and ALT<5*UNL; albumin >=2 g/dL;
4) Normal kidney function: Ccr >=70ml/min/1.73 m2 or normal Scr according to age and gender;
5) Normal coagulation function: INR<1.4; APTT<34s;
6) Volunteer to participate in the clinical trials and sign the informed consent form.Being able to be followed up according to the schedule.

排除标准：

1）曾接受任何抗肿瘤治疗
2）眼底检查提示双眼视网膜母细胞瘤；
3）其他全身状况导致无法接受治疗（例：先天性心脏病，急性或慢性感染性疾病，其他肿瘤等）

Exclusion criteria：

1)Patients with previous anti-tumor treatment
2)Fundus examination show bilateral retinoblastoma
3)Known medical conditions making participation ill-advised (e.g. congenital heart diseaseacute or chronic infectious disease or other malignancy)

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

（1） 病例报告表的填写与数据录入 病例报告表由研究者填写，每个入选病例必须在14天内完成电子数据采集表（EDC）录入 （2） 数据监察 对EDC中存在的疑问，数据管理员将产生疑问解答表（DRQ），并通过临床监查员向研究者发出询问，研究者应尽快解答并返回，数据管理员根据研究者的回答进行数据修改、确认与录入，必要时可以再次发出DRQ。 （3） 数据锁定 全部数据完成录入并解答所有疑问后，由项目负责人、统计分析人员对数据进行锁定。锁定后的数据文件不再做改动。数据锁定之后发现的问题，经确认后在统计分析程序中进行修正。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

(1) Filling in the case report form and data entry The case report form should be filled in by the investigator, and electronic data collection (EDC) of each case must be completed within 14 days. (2) Question monitoring For questions in the EDC, the data administrator will generate a DRQ and send a query to the researcher through the clinical monitor. The researcher should answer and return as soon as possible, and the data administrator will modify the data according to the researcher's answer, confirmation and entry, and can issue DRQ again if necessary (3) Data lock With all the data entered and all the questions answered, the research investigator and statistical analysts will lock the data. The locked data file can no longer be changed. The problems found after the data is locked are corrected in the statistical analysis process after confirmation.
