研究目的：

通过前瞻性随机对照研究，比较不同化疗方案联合放疗在局部晚期鼻咽癌治疗临床效果。  

Objectives of Study：

To compare the clinical effect of treatment for locally advanced nasopharyngeal carcinoma by different chemotherapy regimen combined radiotherapy through a randomised controlled study

药物成份或治疗方案详述：

吉西他滨；紫杉醇；氟尿嘧啶；顺铂 

Description for medicine or protocol of treatment in detail：

Gemcitabine;Paclitaxel;Fluorouracil;cisplatin 

纳入标准：

年龄18～60岁；无化疗禁忌证；病理分型为分化型非角化癌或未分化型非角化癌（WHO分型）；临床分期为Ⅲ、Ⅳａ期（鼻咽癌2008分期）；初治病例，未经化疗、外科治疗、放射治疗；一般状况好，Karnofsky（KPS）评分≥80分。肾功能正常：肌酐清除率≥60ml/min；血常规正常：白细胞≥4 000/μl，血小板≥100000/μl。

Inclusion criteria

Age from18 to 60 years; No contraindication to chemotherapy ; Pathological classification: World Health Organization (WHO) type II or III disease; The clinical stages only consisting of Ⅲ, Ⅳa stage (7th edition AJCC classification of nasopharyngeal carcinoma); All cases not received the chemotherapy, radiation therapy, surgical treatment before randomization, Karnofsky performance score(KPS) ≥80 points. Normal renal function: creatinine clearance rate ≥60 ml/min; normal blood routine test: white blood cells ≥4000/μl, platelets≥100000/μl.

排除标准：

病理为WHOⅠ型或腺癌；年龄＞60岁或＜18岁；KPS 评分＜80 分；肝、肾功能异常者；心脏病、心功能障碍者；有原发病灶和转移灶手术史者；曾经接受过放疗或化疗者。

Exclusion criteria：

Pathological type is adenocarcinoma. Ages>60 or <18 years; karnofsky performance scores <80; first diagnosed NPC with distant metastasis; patients with not-tumor associated death; patients with heart lung liver renal insufficiency; recurrence patients. Surgical history of primary lesion and metastasis .Once received radiotherapy or chemotherapy .
