研究目的：

1. 建立支气管扩张队列人群，分析西南地区支气管扩张患者感染病原分布。 2. 对照组为肺部小结节且排外感染者，支气管扩张患者为试验组，入组受试对象痰培养阳性者，根据临床判定分为定植组与感染组，采用mNGS技术探索感染与定植不同状态下，患者肺部微生态分布差异，并分析患者外周血/BALF宿主宏转录组表达差异。同时患者外周血/BALF行代谢组学检测，明确机体在感染或定植状态下的炎症反应代谢状态是否不同。AI影像组学分析感染和定植不同状态下的影像学差异。 3.痰培养结果提示铜绿假单胞菌者，分析感染组与定植组BALF铜绿假单胞菌宏转录组表达差异。  

Objectives of Study：

1.Estabolish a bronchiectasis cohort to analysis the pathogen distribution of patients with bronchiectasis in the southwest of China, 2.Control group is comprised by small pulmonary nodules and excluded infection. Trial group consist of patients with bronchiectasis. All of the subjects are positive sputum culture. According to clinical judgement divide trial group into infection group and colonization group. Metagenomic next-generation(mNGS) identify pulmonary microecological diversity under different conditions of infection and colonization, as well as analysis host macro transcriptome of patients blood and bronchoalveolar lavage fluid(BALF). Meanwhile, patient's peripheral blood and BALF are used for metabolome detection to identify the difference metabolic status between infection and colonization. Artificial intelligence(AI) Image Omics to analysis the imaging difference between infection and colonization. 3.To analysis BALF Pseudomonas aeruginosa(PA) bacterial macro transcriptomics of patients with PA positive sputum culture in infection group and colonization group.

纳入标准：

健康对照组纳入标准：1. 年龄：18-65岁；2. 胸部CT发现肺部结节。
试验组纳入标准：1. 年龄：18-80岁，经HRCT诊断为支气管扩张；2. 无论感染或定植需痰培养阳性，同时BALF经mNGS验证培养结果。

Inclusion criteria

Inclusion criteria of healthy control group:
1. Age: 18-65 years old;
2. Objects with pulmonary nodules found by chest CT.
Inclusion criteria of the test group:
1. Age: 18-80 years old, diagnosed as bronchiectasis by HRCT;
2. Whether infection or colonization requires sputum culture, BALF culture results were verified by mngs.

排除标准：

健康对照组排除标准：
① 近 4 周内出现急性感染的迹象，包括化脓性痰，高热或白细胞增多；
② 近4 周内有抗生素治疗史；
③ 合并其他慢性呼吸系统疾病；
④ 合并其他系统严重疾病；
⑤ 哺乳或怀孕期；
⑥ 近1月曾因其他肺部疾病入院治疗；中性粒细胞减少症；
⑦ 近1月使用长期免疫抑制剂者（使用激素时间＞7天）
⑧ 不能耐受支气管镜；
⑨ 不签署知情同意书；

试验组排除标准：
① 合并其他慢性呼吸系统疾病；
② 合并其他系统严重疾病；
③ 哺乳或怀孕期；
④ 近1月曾因其他肺部疾病入院治疗；
⑤ 近4 周内有抗生素治疗史；
⑥ 中性粒细胞减少症；
⑦ 近1月使用长期免疫抑制剂者（使用激素时间＞7天）；
⑧ 不能耐受支气管镜；
⑨ 不签署知情同意书。

Exclusion criteria：

Exclusion criteria of healthy control group:
1. There were signs of acute infection in recent 4 weeks, including suppurative sputum, high fever or leukocytosis;
2. Antibiotic treatment history in recent 4 weeks;
3. Other chronic respiratory diseases;
4. Other serious diseases of the system;
5. Lactation or pregnancy;
6. He was admitted to hospital for treatment of other lung diseases in recent 1 month; neutropenia;
7. Those who used long-term immunosuppressive agents in the past 1 month (hormone use time > 7 days)
8. Subjects who could not tolerate bronchoscopy;
9. Subjects who did not sign informed consent.

Exclusion criteria of test group:
1. Other chronic respiratory diseases;
2. Other serious diseases of the system;
3. Lactation or pregnancy;
4. He was admitted to hospital for treatment of other lung diseases in recent 1 month;
5. There was a history of antibiotic treatment in the past 4 weeks;
6. Neutropenia;
7. Those who used long-term immunosuppressive agents in the past 1 month (hormone use time > 7 days);
8. Patients who can not tolerate bronchoscopy;
9. Patients who do not sign informed consent.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

受试者样本采集由昆明医科大学第一附属医院呼吸科执行，血液及肺泡灌洗液的转录组、微生态及病原菌鉴定由广州微远基因科技有限公司检测，血液及肺泡灌洗液代谢组学由广州代谢组由上海厚基生物科技有限公司检测，源数据保存至负责人张剑青处，试验者样本信息录入由各中心采集和核对,并填写病例记录表，然后寄送至负责人张剑青处。张剑青保存所有的源数据，并上传至ResMan网站，并通过该网站进行管理。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

Subjects'specimen collection are implemented by the Department of Respiratory and Critical Care medicine of the First Affiliated Hospital of Kunming Medical University.Transcriptome sequencing、microecological diversity and pathogen detection of blood and BALF are detected by Guangzhou Weiyuan Gene Technology co.LTD .Shanghai Houji biotechnology co.LTD test the metabolome. Each center collects and verifies the specimen information record of all of the subjects and finish the CRF，then send to the research leader, ZhangJianqing.Zhang preserve all of the source datas, upload to ResMan website，and manage datas by it。
