研究目的：

本研究拟通过前瞻性RCT研究，将胃癌患者分为机器人远端胃切除手术（RDG, robotic distal gastrectomy）组、腹腔镜远端胃切除手术(LADG, Laparoscopic distal gastrectomy )组，设置主要终点为两组手术术后30天并发症率。次要终点为术后1年内患者生活质量（QOL, quality of life），为评估患者术后生活质量，采取欧洲EORTC QLQ-C30及STO22中文版进行评分比较，分别于术前，术后3,6,12个月进行评分记录。其他结果分析包括：以手术时间、出血量、淋巴结清扫个数、首次排气时间、住院时间、术后30d病死率等短期疗效指标评价两者的优势及不足，同时以生存率为指标评价两种手术方法在胃癌根治术中的应用价值。  

Objectives of Study：

This study proposed by prospective RCT study, patients with cancer of the stomach can be divided into robot distal gastric resection surgery (RDG, robotic distal gastrectomy) group, the Laparoscopic distal gastric resection surgery (LADG, Laparoscopic distal gastrectomy) group, set the primary end point to two groups of postoperative complication rate 30 days. The secondary end point was QOL, quality of life (QOL, quality of life) within 1 year after surgery. To evaluate the postoperative quality of life of patients, European EORTC qlq-c30 and Chinese version STO22 were used for scoring comparison, and scores were recorded before surgery and 3,6 and 12 months after surgery, respectively. Other results included: the advantages and disadvantages of the two were evaluated by short-term efficacy indexes such as operation time, blood loss, number of lymph nodes dissected, first exhaust time, length of stay, and case fatality 30 days after the operation, and the application value of the two surgical methods in radical gastrectomy was evaluated by survival rate.

纳入标准：

① 病理确诊的进展期胃腺癌患者
② 年龄：18 岁≤年龄≤80 岁
③ 根据AJCC/UICC第8版:术前肿瘤临床分期 cT2-4aN0-3M0
④ 无明显手术禁忌症
⑤ ASA 评分≤Ⅲ级
⑥ 患者知情同意
⑦东部合作肿瘤小组（ECOG）评分0-1分
⑧术前评估肿瘤位于胃部的中下1/3

Inclusion criteria

1.Pathologically diagnosed patients with advanced gastric adenocarcinoma
2.Age: 18 years ≤ age ≤80 years
3.According to AJCC/UICC version 8: cTNM stage: cT2-4aN0-3M0
4.Without surgical contraindications
5.ASA score Ⅲ or less
6.Signed informed consent
7.The eastern cooperative tumor group (ECOG) score : 0-1
8.Preoperative evaluation showed that the tumor was located in the middle and lower 1/3 of the stomach

排除标准：

① 严重精神障碍
② 怀孕或哺乳期妇女
③ 既往上腹部手术史（腹腔镜胆囊切除术除外）
④ 既往行胃切除、内镜下黏膜切除或内镜黏膜下剥离术，残胃癌
⑤ 既往 6 个月内不稳定心绞痛或心肌梗死史
⑥ 术前进行过放化疗
⑦ 既往 5 年内其他恶性肿瘤史（除外甲状腺乳头状癌）
⑧ 胃周围转移淋巴结≥3cm
⑨ 同时需行其他手术的患者
⑩严重呼吸道疾病（FEV1 <50％）
⑾研究前1个月内持续进行全身类固醇治疗

Exclusion criteria：

1.Serious mental disorder
2.Women with pregnant and suckling period
3.Previous history of upper abdominal surgery (except laparoscopic cholecystectomy)
4. Gastric resection, endoscopic mucosal resection or endoscopic submucosal dissection, gastric stump cancer
5.History of unstable angina pectoris or myocardial infarction within the previous 6 months
6.Preoperative chemoradiotherapy
7.History of other malignant tumors within the past 5 years (except thyroid papillary carcinoma)
8.Metastatic lymph nodes around the stomach ≥3cm
9.Patients requiring other operations
10.Severe respiratory disease (FEV1 < 50%)
11.Study within a month before the ongoing systemic steroid treatment

采用分层区组随机来减少由于病情或治疗有关的特定因素（例如性别、年龄、病情轻重）在两组中分配不均匀而引起的不平衡或偏倚。本研究采用中心动态分层策略，随机化序列通过使用SAS 9.3版本，采取Pocock-Simon最小化方法产生，后根据参加中心，患者年龄，临床TNM分期，免疫组化类型（印戒细胞癌与否）进行分层分析。各参加本研究的单位均需向中心组织提交患者相应信息，以便进行随机化。信息及建议治疗方式将由随机中心下发至各个研究中心。
