研究目的：

（1）基于国内外相关研究证据及临床护理实践经验，系统整合脑卒中后患者肌力恢复相关护理措施，构建以功能评估、活动指导、安全管理及健康教育为核心的阶梯式康复护理方案。 （2）依据患者不同肌力水平及功能恢复阶段，明确分层护理目标、护理干预内容及实施流程，形成具有连续性、规范性及可操作性的康复护理管理路径。 （3）通过临床初步应用，评价该阶梯式康复护理方案在改善脑卒中后患者肌力水平、运动功能及日常生活活动能力中的可行性与应用效果，为脑卒中康复护理实践提供参考依据。  

Objectives of Study：

(1) Based on domestic and international research evidence and clinical nursing practice, we systematically integrated nursing interventions for muscle strength recovery in post-stroke patients and constructed a stepped rehabilitation nursing program with functional assessment, activity guidance, safety management, and health education as its core components. (2) According to patients’ different muscle strength levels and functional recovery stages, stratified nursing goals, nursing intervention contents, and implementation procedures were clearly defined, thereby forming a rehabilitation nursing management pathway characterized by continuity, standardization, and operability. (3) Through preliminary clinical application, the feasibility and effectiveness of this stepped rehabilitation nursing program in improving muscle strength, motor function, and activities of daily living in post-stroke patients were evaluated, with the aim of providing a reference for clinical rehabilitation nursing practice in stroke.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

1.经CT或MRI检查确诊为脑卒中（脑梗死或脑出血）的患者； 2.年龄≥60岁； 3.经评估存在肌力下降或肌力减退风险者； 4.生命体征平稳，病情处于恢复期或康复期，具备基本沟通与配合能力； 5.自愿参与本研究并签署知情同意书。

Inclusion criteria

1. Patients diagnosed with stroke (cerebral infarction or cerebral hemorrhage) confirmed by CT or MRI; 2. Age >= 60 years; 3. Assessed as having decreased muscle strength or being at risk of muscle strength decline; 4. Stable vital signs, in the recovery or rehabilitation phase, with basic communication and cooperation abilities; 5. Voluntary participation in this study and signed informed consent.

排除标准：

1.合并严重精神障碍或认知功能严重受损，无法配合评估及护理干预者； 2.合并严重心、肝、肾等重要脏器功能衰竭者； 3.处于疾病急性不稳定期或病情持续进展者； 4.存在严重运动禁忌或不能耐受康复训练者； 5.预计研究期间无法完成随访或主要结局指标评估者。

Exclusion criteria：

1. Patients with severe mental disorders or severely impaired cognitive function, who are unable to cooperate with assessment and nursing interventions;
2. Patients with severe failure of vital organs such as heart, liver, or kidney;
3. Patients who are in the acute unstable phase of the disease or whose condition continues to progress;
4. Patients with severe contraindications to exercise or who cannot tolerate rehabilitation training;
5. Patients who are expected to be unable to complete follow-up or assessment of primary outcome measures during the study period.
