评估症状性重度AS患者TAVR术后主动脉根部解剖3D打印个体化模型冠状动脉入路的可及性及其操作难度  

Objectives of Study:

Evaluate the accessibility and operational difficulty of the coronary artery approach in the 3D printed individualized model of the aortic root anatomy for symptomatic severe AS patients after TAVR surgery

纳入标准:

1. 植入的人工瓣膜位置无明显异常; 2. 因重度AS病变而已经完成了TAVI并且有术后CT的患者; 3. TAVR术后预期寿命大于1年。

Inclusion criteria

1. The position of the implanted artificial valve shows no obvious abnormalities; 2. Patients who have undergone TAVI due to severe AS and have undergone postoperative CT; 3. The expected lifespan after TAVR is greater than one year.

排除标准:

1. 首次TAVR时同期植入两个或以上THV; 2. TAVR术前或术后未行主动脉根部增强MSCT检查; 3. 主动脉根部增强MSCT图像质量差,例如存在明显伪影、造影剂对比度过低或图像噪声过重等情况,影响数据分析; 4. 无法下载多层螺旋CT(multislice computed tomography,MSCT)DICOM格式数据以进行离线分析。

Exclusion criteria:

1. Concurrent implantation of two or more thoracoabdominal valves (THV) during the first TAVR procedure; 2. No enhanced MSCT examination of the aortic root was performed before or after the TAVR procedure; 3. The quality of the enhanced MSCT images of the aortic root is poor, such as presence of obvious artifacts, low contrast of the contrast agent, or excessive image noise, which affects data analysis; 4. Unable to download the DICOM format data of multi-slice computed tomography (MSCT) for offline analysis.

测量方法:

选取上一部分具有代表性入路的患者若干,约100例-150例,每个3D打印模型均为患者真实解剖结构及THV几何形状的精确1:1尺寸复制品。为验证冠状动脉通路的可行性与操作难度,由具有丰富经验的介入心脏病专家对所有3D打印模型的左右冠状动脉实施模型模拟冠脉介入操作并评价。在模型模拟冠脉介入操作过程中,操作者被要求按照常规操作规范,使用常规介入导管,导丝完成操作。若初始操作未成功,后续导管选择或插管策略

After the verification of the coronary artery approach on the 3D model was completed

Measure method:

Select several representative patients from the previous part, approximately 100 to 150 cases, with each 3D printed model being an exact 1:1 size replica of the patient's true anatomical structure and the geometric shape of the THV. To verify the feasibility and operational difficulty of the coronary artery approach, experienced interventional cardiologists evaluated and assessed all the 3D printed models by performing model-simulated coronary intervention operations on the left and right corona
