研究目的：

分析心血管外科术后气管插管患者OMPI的发生特征及相关危险因素，构建预测模型，为早期识别高风险患者和制定针对性预防策略提供理论依据。通过模型构建与验证，指导临床开展预见性护理，降低OMPI发生率，减轻患者痛苦，缩短ICU住院时间，提升护理质量。  

Objectives of Study：

To analyze the occurrence characteristics and related risk factors of OMPI in tracheally intubated patients after cardiovascular surgery, and establish a prediction model, so as to provide theoretical evidence for early identification of high-risk patients and formulation of targeted preventive strategies. Model establishment and verification can guide clinical predictive nursing, reduce the incidence of OMPI, relieve patients' discomfort, shorten ICU length of stay and improve nursing quality.

纳入标准：

1.年龄 ≥ 18岁、≤85岁；ICU停留 ≥ 24小时;
2.在本院接受心血管外科手术，术后转入CVICU;
3.术后留置经口留置气管导管并行机械通气；
4.患者或其法定代理人知情同意并已签署知情同意。

Inclusion criteria

1.Aged between 18 and 85 years old; ICU stay duration >= 24 hours; 2.Underwent cardiovascular surgery in our hospital and were transferred; to CVICU postoperatively; 3.Received postoperative orotracheal intubation and mechanical ventilation; 4.The patient or their legal representative has been fully informed and signed the informed consent form.

排除标准：

1.入ICU时已发生OMPI;
2.存在口腔基础疾病影响评估;
3.术前气管插管患者;
4.参与其他可能干扰本研究研究结果的临床试验者或临床资料不完整。
5.患者或其家属要求退出研究；
6.在研究观察期内，因病情变化非计划性拔除气管插管后再次插管（仅分析首次插管数据）；
7.任何研究者判断不适合继续参与本研究的情况。

Exclusion criteria：

1.OMPI occurred upon admission to ICU；
2.Presence of underlying oral diseases interfering with evaluation；
3.Patients with preoperative tracheal intubation；
4.Participants enrolled in other clinical trials that may interfere with the study results or those with incomplete clinical data.
5.The patient or their family members requests to withdraw from the study；
6.During the study observation period, unplanned extubation followed by reintubation occurred due to changes in the patient's condition. Only data from the initial intubation will be analyzed；
7.Any circumstances where the investigator deems the participant unfit to continue in this study.

1 数据管理 每日9:00与16:00行口腔护理时评估口腔黏膜状况，记录至拔管、转科、出院或死亡为止。 1.1 数据采集方法与工具： 所有数据均使用医院统一的病例报告表进行采集。 1.2 数据记录与保存： 数据由经过统一培训的研究协调员从医院各信息系统中提取填入统一制定的Excel数据录入表，并录入病例报告表。所有电子数据将存储在陆军军医大学第二附属医院本地的安全服务器上，实行严格的权限管理和登录审计。研究数据将进行匿名化处理，使用唯一的研究编号替代患者个人信息。连接研究编号与患者身份信息的“代码表”将作为保密文件，与研究数据分开存储。数据保存期限将遵循国家关于医学研究数据和人类遗传资源管理的相关法律法规。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1 Data ManagementOral mucosal conditions were assessed during oral care at 9:00 and 16:00 every day until extubation, ward transfer, hospital discharge or death.1.1 Data Collection Methods and ToolsAll data were collected using unified hospital case report forms.1.2 Data Recording and StorageUniformly trained research coordinators extracted data from various hospital information systems, filled in standardized Excel entry sheets and completed case report forms. All electronic data were stored on local secure servers of The Second Affiliated Hospital of Army Medical University with strict authority management and login audit.All research data were anonymized, and unique study IDs were used to replace patients’ personal information. The code sheet linking study IDs and patient identity information was kept as confidential documents and stored separately from research data. The data retention period complied with national laws and regulations concerning medical research data and human genetic resource administration.
