研究目的：

本研究旨在建立江苏省多中心肾移植专病队列，通过系统收集与整合长期随访数据，识别影响移植肾长期存活的早期预警指标及关键预测因素，评估供肾的远期预后，为供肾质量评估与分配决策提供循证依据，推动肾移植临床管理从经验性决策向基于多维度数据的精准评估与个体化干预转变  

Objectives of Study：

We are establishing a multicenter kidney transplantation cohort in Jiangsu Province. By systematically collecting and integrating long-term follow-up data from multiple centers, we hope to identify early warning indicators and key predictors of long-term graft survival, evaluate how donor kidneys perform over time, and generate evidence to support donor kidney quality assessment and allocation decisions. Our ultimate goal is to move clinical management in kidney transplantation away from experience-based decisions and toward data-driven precision assessment and individualized intervention.

纳入标准：

1.所有在合作肾移植中心接受肾移植手术的患者，供肾类型包括DCD供肾和活体供肾。

Inclusion criteria

1.Patients who undergo kidney transplantation at any participating center, with donor kidneys from either donation after circulatory death (DCD) or living donors.

排除标准：

1.无法理解研究内容或不愿签署知情同意书者；
2.因精神、认知或地理距离等客观因素无法配合长期随访者。

Exclusion criteria：

1. Individuals who are unable to understand the study content or unwilling to provide informed consent.
2. Individuals who are unable to comply with long-term follow-up due to objective factors such as psychiatric disorders, cognitive impairment, or geographic distance.
