研究目的:

本研究从时间动态角度量化分析股骨头坏死的病情演变规律,主要探究负重区硬化带与坏死区信号强度比值(SIR)对股骨头坏死塌陷发生时间的预测作用,明确其能否作为预测股骨头塌陷的独立影像学指标;同时分析 SIR 与距塌陷发生时间的纵向相关性,探寻塌陷前 SIR 的动态变化趋势及关键时间窗口,构建 4 个月内股骨头塌陷短期风险预测模型,确定 SIR 临床预测最佳截断值,并在校正 JIC 分型等传统预后因素后,进一步验证 SIR 的独立预测价值,为股骨头坏死早期干预及短期塌陷风险预判提供客观影像学依据。  

Objectives of Study:

This study quantitatively analyzed the evolution pattern of osteonecrosis of the femoral head from a temporal dynamic perspective. The main objective was to explore the predictive effect of the ratio of signal intensity (SIR) between the hardened zone and the necrotic zone in the weight-bearing area on the time of femoral head collapse, and to clarify whether it could serve as an independent imaging indicator for predicting femoral head collapse. At the same time, the longitudinal correlation between SIR and the time of collapse occurrence was analyzed to explore the dynamic change trend of SIR before collapse and the key time window. A short-term risk prediction model for femoral head collapse within 4 months was constructed, and the optimal cutoff value of SIR for clinical prediction was determined. After correcting for traditional prognostic factors such as JIC classification, the independent predictive value of SIR was further verified to provide objective imaging evidence for early intervention of femoral head necrosis and short-term risk prediction of femoral head collapse.

纳入标准:

1.确诊为股骨头坏死(ONFH,ARCO I–III 期或 JIC 分型明确);
2.具有完整随访资料,并最终发生塌陷;
3.塌陷前 0–12 个月内存在规律次影像学检查(X 线正位和蛙位);
4.影像质量满足灰度分析要求;
5.可明确标记影像与塌陷时间间隔;

Inclusion criteria

1.Diagnosed with osteonecrosis of the femoral head (ONFH, ARCO I–III stages or JIC classification is clear);
2.With complete follow-up data and the final occurrence of collapse;
3.Before the collapse, there were regular imaging examinations (X-ray anteroposterior and frog position) within 0-12 months.
4.The image quality meets the requirements of the grayscale analysis.
5.The imaging can be clearly marked with the time interval between the image and the collapse.

排除标准:

1.既往接受股骨头手术治疗(如髓芯减压、截骨等);
2.影像学质量差或无法进行 ROI 测量;
3.合并严重骨关节疾病(如感染、肿瘤);
4.随访信息不完整或塌陷时间不明确;

Exclusion criteria:

1.Having previously undergone surgical treatment for the femoral head (such as core decompression, osteotomy, etc.);
2.Poor imaging quality or inability to perform ROI measurement;
3.Combine severe bone and joint diseases (such as infections, tumors);
4.Incomplete follow-up information or unclear collapse time;
